Location BBC Bronson Battle Creek Title Bronson Battle Creek Registered Nurse MICU PRN (Variable Shifts) Under general supervision and in accordance with the policies, procedures and guidelines established within the organization, the unit, and the Michigan Public Health Code, the registered nurse manages the care delivery for patients in the organization throughout the length of stay to achieve quality outcomes and timely discharge by the following: utilizing the nursing process of assessment, planning, diagnosing, implementation and evaluation; coordinating the care provided by other professionals; delegating selected acts, tasks or functions that fall within the scope of professional nursing practice and the abilities of patient care assistants and universal service assistants; providing selected aspects of physical care within the scope of professional nursing practice as appropriate given the patient's level of acuity and complexity of care required; and documenting and communicating during each tour of duty the patient's response to care provided, consumption of resources and anticipated needs. Employees providing direct patient care must demonstrate competencies specific to the population served. Associate's degree in Nursing required Bachelor's degree in Nursing preferred, must obtain within 8 years of hire Licensed Registered Nurse in good standing for the State of Michigan As of December 29, 2013, any nurse hired by Bronson who does not hold a BSN (or higher nursing degree) will be given 8 years from date of hire date to obtain. If, after 8 years, the nurse has not met this requirement, employment will be terminated. BLS certification required by completion of core orientation Critical care or ALS is required for some RNs, unit specific RNs in Labor & Delivery and Mother/Baby/Antepartum at BMH & BBC Specific : The Electronic Fetal Monitoring Certification through PQF or NCC is required within 6 months of completing intermediate fetal monitoring • Ability to read, interpret, and analyze data from various computer systems and equipment in order to set an appropriate plan of action • Must be able to constantly communicate both verbally, in writing and electronically Work which produces high levels of mental/visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time, the operation of and full attention to a personal computer or CRT between 40 and 70 percent of the time. Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds), and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment, and other objects. Registered Nurse - Ambulatory BFCC / Pediatric Specialty Clinics

Overview Assumes responsibility and accountability for patient screenings; triage; pain management; falls prevention, nutritional and psychosocial, discharge planning and other interventions to restore, maintain or promote patients’ physical health. Utilizes the nursing process in the provision of direct patient care. Assists all patients, families/significant others through the healthcare system by acting as a patient advocate and navigator. Responsible and accountable for prescribing, delegating, and coordinating all care provided to assigned patients. Executes medical plan of care, at the direction of the treating clinician, in accordance with licensure of a Registered Professional Nurse. Utilizes nursing diagnoses to treat human responses to actual and potential health problems through such services as case finding, health teaching, health counseling, and provision of care supportive or to restorative of life and well-being. Supervises patient care provided by Licensed Practical Nurses (LPN), Medical Office Assistants (MOAs), Nursing Attendants (NAs), technicians, and supplemental staff. Supports patients’ self-management of disease and behavior modification interventions. Coordinates continuity of patient care with external healthcare organizations and facilities, including the process from admission to discharge and referrals from the primary care provider to the specialty care provider. Daily, weekly and monthly checks of all logs including refrigerators, medications, testing equipment as per established protocols. Coordinates the overall plan of care to ensure implementation of nursing interventions and consistency with medical regimen. Assures the plan of care integrates age-specific care needs (e.g. pediatrics, geriatrics) as defined in the unit scope of care. Accepts responsibility and accountability for nursing interventions and outcomes based on professional clinical judgments. Demonstrated ability to communicate effectively, manage stressful situations appropriately and facilitate interpersonal relationships with other disciplines. Ensures that patients receive timely and appropriate care within the delivery system and assists with facilitating internal and external referrals. Communicates with pharmacies as well as insurance companies related to patient treatment plans and approvals of medications and/or procedures/Diagnostic Imaging. Collaborates with medical providers in identifying patient’s educational needs and provides resources to meet these needs. Performs vital signs: T-P-R, O2 Sat, as required and weight, height and blood pressure, as needed. Performs diagnostic testing: EKG, point of care testing, as needed. Administration of medications and immunizations at the direction of the physician. Orders all vaccines, including Vaccines for Children Program and documenting pediatric immunizations in City wide Immunization Registry. Collects various screening information including: Falls, Nutritional, Psychosocial and Pain Assessments, as required and documents findings. Assumes role of patient advocate and conforms to policies and regulations governing patient rights, confidentiality and the HIPAA Privacy & Security Rules. Participates in Performance Improvement/Quality Assurance activities and demonstrates initiative in identifying and communicating performance improvement/quality assurance problems and issues. Participates in appropriate professional and community organization activities. Obtaining prior authorizations for (this list is not all inclusive): MRI, CAT Scans, EMG, Video Ambulatory and Ambulatory EEGs, medications, diagnostic testing and procedures, as applicable to insurance carriers. Includes review of Provider documentation to ensure tests/procedures are included for insurance purposes. Ensure lab testing has been ordered as per specialty clinic guidelines, i.e., every visit or every quarter including Genetic testing, Specialized Hematology, Renal and Gastroenterology services Schedule external appointments for (this list is not all inclusive): Early Intervention; PT/OT; Neurosurgery; Pediatric services: Orthopedics, Eye, Genetics and Psychiatry, as applicable. Establish routine communication with foster care agencies/Nursing staff; ACS; School Nursing, Psychiatry and Social Work staff, internal and external resources. Completion of school, foster care agency and early intervention forms are completed in a timely manner. All other duties as assigned. Qualifications Education / Experience: Internal & 1199 SEIU: Graduate of accredited nursing program(s). Bachelor's Degree in Nursing preferred. Description The per diem Midwife is responsible for the primary care and management of patient's health, including obstetric and gynecological care. Both low risk and high-risk pregnancies are managed as outlined within practice guidelines. Develops and supports excellence in the clinical practice and patient care. Maintains the Maternal Child Health Services mission, philosophy, and policies as well as the standards and practices of the Medical Center. Responsibilities 1. The midwife performs functions as an integrated member of the Department of OB/GYN and Nursing. The midwife will: Coordinate prenatal care to the caseload of patients in the ambulatory care setting. 2. Take pertinent history. Perform physical & pelvic examination. Obtain laboratory and diagnostic tests. Provide counseling & teaching as needed. 3. Identify risk factors and develop appropriate plan of management. Manage the care of selected patients admitted to the inpatient unit-based on unit protocols and with the collaboration of the attending physician. 4. Obtain physician consultation for plan of management for any medical or obstetrical complications not addressed in departmental protocols. Performs functions as identified in the Midwife Scope of Practice. 5. Provide Intra-partum Care as follows: Evaluate the status of labor and admits patient to the labor floor. Perform admission physical examination. 6. Perform venipuncture and starts intravenous infusions if nursing staff is unavailable. Obtain laboratory and diagnostic tests & evaluates results. Evaluate the progress of labor by vaginal examination. Make decisions regarding ambulation of the patient during labor. Request conduction anesthesia. 7. Perform amniotomy as indicated. Apply external fetal monitor & tocodynamometer. Apply internal fetal scalp electrode & pressure catheter. Evaluate fetal monitor tracing & consults with physician if abnormalities are present. Make decisions regarding the position(s) of the patient during second stage of labor. Make decisions regarding performance of delivery in the birthing room or transfer to the delivery room. 8. Utilize local infiltration or pudendal block. Perform & repairs episiotomies. Conduct spontaneous deliveries of cephalic presentations. Obtain specimen of cord blood for cord gases when appropriate. Manage the third stage of labor. Perform cervical & vaginal inspection. Repair minor vaginal & perineal lacerations. Take responsibility for the management of the uncomplicated patient in recovery room. 9. Decide when the normal infant should be transferred to the nursery. Request attendance of pediatrician at delivery when neonatal problems are anticipated. Order intermittent fetal monitoring based on patients risk status. Perform immediate appraisal of the newborn. Perform newborn physical examination. Sign birth certificate of delivery (operative delivery requires physician signature). Coordinate patient care in triage room on L&D. Attend and participate in L&D board rounds. 10. Participate in the care of patients during labor & delivery in consultation with attending physician: Perform prostaglandin insertions for cervical ripening. Manage Oct(s) after evaluation of the patient by the physician. Insert intrauterine pressure catheters. Perform & monitor amnio infusion. Monitor patient on IV oxytocin & regulates oxytocin infusion for induction or stimulation of labor following the initial stabilization & monitoring of the patient by the physician. Conduct spontaneous deliveries of these patients under supervision of the attending. 11. Manage the labor & conduct the deliveries of patients who have been approved by the physician for vaginal birth after cesarean section (VBAC). Repair cervical or extended vaginal lacerations under the supervision of the attending. Repair third & fourth degree extensions under the direct supervision of the attending. Participate as first assist on C-sections and/or BTL. Insert Foley catheter bulb for induction of labor. Perform misoprostol (Cytotec) insertions for labor induction. Order appropriate drugs to treat hemorrhage during third stage of labor per department policy for Post-Partum Hemorrhage. Manually remove placenta. Perform exploration of uterus. Perform selected patient high risk delivery at discretion and under the supervision of the physician. 12. Provide postpartum care as follows: Make daily rounds, examine patients, orders & interpret laboratory results, diagnostic tests & monitor the postpartum course. Provide postpartum teaching & counseling. Prepare patients for postpartum bilateral tubal ligations. Manage patients who have had cesarean sections or postpartum bilateral tubal ligations based on unit protocols & under the supervision of the attending physician. 1) Write medication & non-medication orders 2) Remove surgical sutures & staples Perform fever work-ups for patients with postpartum febrile morbidity, as per unit protocols & consults the physician for management assistance. Make appropriate discharge plans. Obtain physician consultation for plan of management for any medical or obstetrical complication. Obtain informed consent for circumcision of male infant. Job Description Under the supervision of the Administrative Nurse (AN) or Charge Nurse, the Registered Nurse (RN) is responsible for providing skilled and comprehensive nursing care to radiation oncology patients with a wide variety of oncological and/or disease-related problems seen for consultation, treatment, and follow-up. Through application of the nursing process, the RN provides direct care and implements plans for continuity and coordination of patient care with other members of the patient’s health care team.
