Job Description The Department of Ambulatory Care Services at SUNY Downstate Health Sciences University is seeking a full-time Assistant Director of Nursing/Nurse Practitioner, Suite G OB-GYN. The successful candidate will: Provide primary care services to individuals and families; this will include care for incumbents own panel of patients, as well as seeing walk-in patients of the practice as needed. Document all care provided in the patients medical records. Develop further, maintain/manage patient care panel with participation in managed care programs and other direct patient care programs as applicable. On a daily bases, review all laboratory and radiology reports to identify abnormal results for urgent follow up; ensure that patients are contacted to arrange additional care and that their primary providers are notified. Proficiency of the English language including the ability to speak, read, write, and comprehend the language with minimal to no difficulty • Valid CPR certification General Duties and Responsibilities: GENERAL • Provides nursing care as prescribed by physician/health care professional in accordance with the legal scope of practice, any Board of Licensing restrictions, and within established standards of care, policies, and procedures • Administers professional services and provides care consistent with allowing residents to attain or maintain his or her highest practicable physical, mental, and emotional well-being. Provides clinical data and observations to contribute to the nursing plan of care CLINICAL • Admits, transfers, and discharges residents in accordance with policy and procedure • Orients new residents/families as necessary • Gives/receives nursing reports upon beginning and ending of assigned shift. Reviews daily patient report and assignments with Nursing Assistants upon the beginning of assigned shift • Assures that the unit area is maintained in a clean and safe manner for resident comfort and convenience. • Conducts daily resident rounds to observe the resident’s physical and emotional status • Collects clinical data and reports significant clinical findings according to policy • Performs manual and technical skills according to the scope of practice under the direction of a Registered Nurse • Records care information accurately, timely and concisely. Completes all required documentation including resident observations, interventions, and patient response(s) in the medical record in accordance with policy • Assists in developing, reviewing, revising, and updating resident Plans of Care as indicated. Contributes to the evaluation of the patient’s progress towards specific goals and the adjustment of the nursing plan of care as necessary • Receives and transcribes orders accurately from attending/alternate physician. • Assists in making rounds with the attending physician, as necessary. • Prepares/administers medications as ordered by the physician and within the legal scope of practice. • Completes medical treatments as indicated and ordered by the physician. • Evaluates response and report adverse reactions to medication or treatments in accordance with the policy. • Assures that narcotic and controlled drugs are secure, inventory counts are accurate and narcotic register documentation is timely and accurate • Orders prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician. • Notifies the attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within established policies. • Notifies the attending/alternate physician of resident incidents/unusual occurrences. Completes Accident/Incident Report forms as indicated and documents relevant information in the resident’s medical record as outlined in established policies and procedures. Initiate investigation of accidents and unusual occurrences • Informs RN Supervisor or RN in charge of the facility of significant changes in resident condition, unusual occurrences, incidents, or accidents as indicated and per facility policy and procedure • Attend various facility meetings (care conferences, stand-up meetings, staff meetings, Medicare meetings, discharge plan meetings, etc.) as required. • Reports allegations of abuse or unusual occurrence per State and Federal Regulations and facility policy. Our comprehensive heart failure program provides APPs the opportunity to practice at the top of their license while contributing to life-saving advanced cardiac therapies. Position Summary St. Francis Hospital & Heart Center is seeking a highly skilled Inpatient Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Comprehensive Congestive Heart Failure Program. The APP will provide advanced, evidence-based care to patients with acute and chronic heart failure across all stages of disease severity, including complex and advanced therapies. This role practices collaboratively with board-certified Heart Failure Cardiologists, cardiac surgeons, intensivists, nurses, and multidisciplinary specialists to deliver safe, effective, and compassionate inpatient care. Program Overview The Center for Advanced Cardiac Therapeutics offers comprehensive management for patients with weakened heart muscles and advanced heart failure, including expertise in: Advanced medical heart failure therapy LVAD and VAD management ECMO support Cardio-oncology Cardiac amyloidosis and sarcoidosis Pulmonary hypertension Cardiogenic shock Structural heart disease (in partnership with the Heart Valve Center) Key Responsibilities Perform comprehensive inpatient evaluations of patients with acute decompensated and chronic congestive heart failure. Diagnose and manage systolic and diastolic heart failure. Develop and implement individualized treatment plans. Manage guideline-directed medical therapy (GDMT). Provide care for patients requiring advanced therapies including LVAD, VAD, and ECMO. Assist in the management of cardiogenic shock and complex cardiac conditions. Monitor laboratory results, imaging, and hemodynamic data. Order and interpret diagnostic tests. Participate in multidisciplinary rounds. Assist in advanced cardiac therapeutic interventions. Participate in LVAD patient evaluation and inpatient management. Support ECMO and mechanical circulatory support management (in collaboration with intensivists and cardiologists).
