Overview NaphCare is hiring a MAT Nurse Practitioner to join team of medical professionals in the Pueblo County Jail in Pueblo, CO . As a MAT Nurse Practitioner , you will work alongside correctional officers and other medical professionals to provide support to the patients within the facility struggling with substance abuse. This is a unique position where you can make a difference to this diverse population of patients with complex needs. Responsibilities The MAT Nurse Practitioner plays a critical role in our continued mission to provide quality care that protects the rights, dignity, and health of our unique patient population. Duties and responsibilities may include and are not limited to: Provide comprehensive Medication-Assisted Treatment (MAT) services for incarcerated individuals with Opioid Use Disorder (OUD), with a primary focus on buprenorphine/naloxone (Suboxone). Conduct initial and ongoing clinical assessments for OUD, including substance use history, withdrawal evaluation, psychiatric and medical comorbidities, and readiness for treatment. Initiate, continue, adjust, and monitor Suboxone therapy in accordance with evidence-based guidelines, DEA regulations, and correctional health policies. Manage induction, stabilization, maintenance, and tapering phases of MAT as clinically indicated. Collaborate closely with correctional healthcare staff, custody staff, mental health providers, and substance use counselors to ensure coordinated, safe, and effective care. Monitor patient adherence, response to treatment, and potential diversion risks within a correctional setting. Provide patient education on OUD, MAT, relapse prevention, overdose risk, and continuity of care upon release. Participate collaboratively with an interdisciplinary team to create individualized treatment plans that address medical, behavioral health, and social determinants of health. Ensure accurate, timely, and compliant documentation in the electronic health record, including MAT-specific reporting and regulatory requirements. Participate in discharge planning and continuity-of-care coordination, including referrals to community-based MAT providers upon release. Maintain compliance with state, federal, and facility regulations related to controlled substances, MAT delivery, and correctional healthcare standards. Participate in quality improvement initiatives, clinical audits, and program development related to OUD and MAT services. Stay current with evolving best practices, clinical guidelines, and regulatory changes related to OUD treatment and MAT. Qualifications Education / Experience / Requirements Advanced nursing education (Master's or Doctorate) MAT/OUD treatment experience required CARN-AP preferred Current licensure as a Nurse Practitioner Must hold a current unrestricted license as a Nurse Practitioner in Colorado . Current DEA registration An appropriate collaborative agreement with the on-site Physician Must possess an active BLS CPR Certification (AHA required) Be certified in a Board recognized by the American Board of Medical Specialties Knowledge of nursing practice principles and techniques, occupational health and safety standards, infectious disease management, applicable health care laws, regulations, and procedures for direct health care services. Discusses with the attending physician and nursing staff physical and/or emotional findings, which may interfere with care of the patient. Manage all aspects of the patient’s symptoms, including medication/prescription management, conduct family meetings as needed and requested, coordinate care with other members of the IDT through daily rounds, and document care activities and the rationale for the GIP level of care. Responsibility for completion of the MOLST form as required and for the completion of the death certificate of each expired patient. Attend the weekly inter-disciplinary team meetings Work with the hospice access team to facilitate a smooth admission process. Work with the Gilchrist Inpatient Centers inter-disciplinary team to facilitate live discharges from the Center, including completion of transfer/discharge summary, prescriptions, and other discharge related documentation. Provide telephone and afterhours coverage via emedicall for patients at the assigned Center Evaluates patient progress on a continuing basis and in collaboration with the attending physician, and alters treatment modalities as needed. Evaluates nursing, medical, social and educational needs on assigned patients. Writes progress notes for each visit documenting pertinent information regarding patient progress and treatment regime. Writes orders appropriate to the patient's care requirements in compliance with existing contract. Establishes medical diagnoses in compliance with contract. Refers to the physician or designee those problems which require consultation and documents same on progress note. Performs technical functions as covered by the job description and delegation agreement submitted to BPQA and/or MBON, approved NP agreement and in accordance with hospital policy. Documents results of technical functions performed. Reports to physician or designee any abnormal findings related to it. Where You Will Work: 9ST is a 48 bed Adult Moderate Acuity Cardiovascular Surgical unit. Working closely with the CVICU, we provide care to those patients needing specialty services before and after their heart surgery, heart transplant and/or ventricular assist device (VAD). Orientation to our unit is completed in two phases. Phase one focuses on the care of the cardiac surgical patient with phase 2 centering on the more complex needs of the heart transplant, ultrafiltration or VAD patient.
