Responsibilities: Essential Functions Perform primary health care for patients by utilizing the processes of assessment and diagnosis of health status. Obtain patient's medical and history information. Data collection is prioritized according to immediate condition or needs. Perform physical exam and determine patient's mental status. Develop and implement treatment, follow-up and evaluation plans for patient outcomes. Collaborate with physicians and other disciplines as appropriate and provide regular and consistent feedback. Use appropriate assessment techniques & diagnostic information. Performs tests relevant to patient condition and record findings. Order appropriate ancillary tests. Derive diagnosis from assessment data and diagnostic information as data becomes available. Use new or additional data to form a treatment plan. Perform telephone triage to determine patient condition and provides appropriate consultation scheduling appointments when needed. Incorporate research into clinical practice as appropriate. Provides non-urgent, urgent and sometimes emergent healthcare. This may include assessment, consultation, establishing diagnoses and implementation of a treatment plan according to approved practice protocols. Implement interventions consistent with the established plan of care in a safe and appropriate manner. Apply established therapeutic practices to patient's concerns and needs. Administer medications and immunizations using the appropriate method and with physician oversight if applicable. Prescribe medications according to Controlled Substance Registration Certificate issued by the DEA. Generate referrals to appropriate services as needed to further diagnose and/or progress of care. Evaluate patient care to determine response to care/interventions for optimal patient outcome. Evaluate the patient's capabilities and/or limitations related to nursing and medical interventions. Recognize and address patient and family capabilities and limitations (physical and mental). Return patient telephone calls. Monitor outcomes and documents patient response to interventions. Seek out new treatment methods necessary to ensure patient progress. Provide patient and family teaching and education specific to the patient's health care. Proper use of medication and health/illness prevention to ensure adequate knowledge/awareness. Interview and advise specific to patient needs. Demonstrate sensitivity to family regarding need for patient information on a timely basis. Work collaboratively with physicians and other team members to provide appropriate patient education geared to the patient's needs and family's ability to provide care. Use adult learning principles for teaching purposes being careful to check for understanding and being available for follow-up questions. Recommend references and resources such as printed materials, online, community and other information sources to fulfill the family's need for education. Maintain a professional behavior in attitudes, demeanor, and interactions to achieve organization's mission. Communicate effectively with physicians, staff and patients. Responsibilities PRIMARY: Ability to fulfill all responsibilities of the Patient Care Specialist and Scribe position as detailed in the specific Patient Care Specialist and Scribe job description. The ability to maintain friendly, cordial relations with all employees, build and maintain confidence and credibility with all employees. Deliver clinical and administrative support for the team and adhere to all policies and procedures of the company. Ensure that the activities of the Nurse are conducted in a manner that is consistent with overall department expectations and are in compliance with Federal and State regulations, guidelines, and requirements including working knowledge of all health information management issues such as HIPAA. Prepare exam and treatment rooms for patient intake. Triage patients by checking vitals, performing blood draws and giving injections, etc. Observe patients, charting, reporting changes in patients’ conditions, such as adverse reactions to medication or treatment and taking any necessary action. Assist with lab testing and phlebotomy. Assist providers in preparing for minor procedures and physicals. Prepare and maintain supplies and equipment for treatment. Assist patients utilizing a Virtual Medicine Provider. Ensure accurate completion of patient charts in a timely matter and forwarding charts as appropriate on a daily basis. Arrange referrals to a specialist and obtaining pre-authorizations when directed by the Provider. The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in a positive work relationship with customers, co-workers and managers. Perform federal and non-federal DOT, BAT and Drug Screen exams. Welcome new employees on their first day in the clinic and facilitate introductions to team members, clinic tours, and an overview of the clinic processes. Actively assist new employees with learning activities and completing required training. Support new employee training by providing job shadowing, demonstration, and coaching opportunities The ability to perform the physical, use of senses, cognitive, and environmental functions of the position, as specified on the physical demands. SECONDARY: Assist with clerical work such as checking in patients, obtaining and recording demographic, insurance and financial information, with strong attention to detail and in a manner that is professional and caring to our patients. Verify insurance eligibility and relay relevant information on payment policies and billing/collection processes. Work as part of the overall team by answering telephone, taking messages and returning voicemails in a friendly and professional manner *** Additional Requirements and Responsibilities: Level 1: High School Diploma or its equivalent with a current CPR license* and/or at least 1 year of experience as a certified LPN preferred in clinic setting, preferably urgent care Level 2: Certified LPN with a current CPR license* and either (1) 3 years’ experience in relevant role and 1 year of Fast Pace experience in Nurse role or (2) 4 years’ experience in relevant role. Or Certified RN with a current CPR license and either 1 years’ experience in relevant role and 1 year of Fast Pace experience in Nurse role or (2) 2 years’ experience as Nurse. Level 3: Certified LPN with a current CPR license* and either (1) 5 years’ experience in relevant role and 2 years of Fast Pace experience in Nurse role or (2) 6 years’ experience in relevant role OR Certified RN with a current CPR license and either 3 years’ experience in relevant role and 1 year of Fast Pace experience in Nurse role or (2) 4 years experience as a Nurse. Level 4: Certified RN with a current CPR license* and at least 10 year as a Nurse and 1 year of Fast Pace experience. Level 5: Certified RN with a current CPR license* and at least 15 years as a Nurse and 2 years of Fast Pace experience. Level 6: Certified LPN with a current CPR license* and either (1) 5 years’ experience in relevant role and 2 years of Fast Pace experience in Nurse role or (2) 6 years’ experience in relevant role OR Certified RN with a current CPR license and either 3 years’ experience in relevant role and 1 year of Fast Pace experience in Nurse role or (2) 4 years experience as a Nurse. Additional Responsibilities: Administration duties, including but not limited to scheduling, ordering, training, and other duties assigned by Clinic and or Regional Director *Receive and active within 7 days of hire.
