The Advanced Practice Registered Nurse (APRN) provides a comprehensive health assessment and evaluation for patients in the clinic under the supervision of the medical director. This position provides health care services to patients under direction and responsibility of a physician including assessing, diagnosing, prescribing, treating, and educating patients. This position works closely with physicians and other clinicians in a team approach to patient care. The APRN orders and interprets appropriate tests i.e. labs, EKG, and Xrays under physician supervision. Orders or performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with physicians for evaluation of illness. Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenance. Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures. Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care. Collaborates with physicians in managing acute and long-term medical needs of patient. Provides monitoring and continuity of care between visits according to treatment plan including triaging patient messages. Job Summary The Medical Assistant on the primary care float pool works to support direct patient care at any of our 15 primary care locations. Primary location will be Foxboro, MA with 1-2 shifts per week at this locaiton. The primary care practices care for a variety of patients *-including, Family Medicine, Community Health Centers, Pediatric Care and Adult Internal Medicine. Qualifications The Medical Assistant (MA) is an integral member of the healthcare team and is accountable for patient care as assigned by a licensed independent provider (LIP: MD, NP, PA), a Licensed Practical Nurse (LPN) or a Registered Nurse (RN). The MA participates in the direct and indirect care of patients, health maintenance, patient instruction, collaborative planning and rehabilitation within scope of practice and consistent with his/her education and training. The Medical Assistant may be present in secure medication storage areas and is allowed incidental access to medications. Developing relationships with patients, families and colleagues, the MA functions as an interdisciplinary team member who provides quality patient care and contributes toward the attainment of important goals for the patient, the practice and the organization. Reinforces patient instruction as directed by LIP/LPN/RN. - Participates in organizational and unit-based quality improvement initiatives and compliance readiness. - Participates in telephone screening of patients; may provide instruction to patient when directed by the LIP/LPN/RN. - Participates in prescription management tasks including pending of new prescriptions and renewals requiring LIP approval, prior authorizations and medication reconciliation. - Follows approved clinical practice guidelines (CPG’s) and workflows, pends appropriate orders for radiology, laboratory testing and other care needs requiring LIP authorization. - Performs a variety of tasks, including those that may be facilitated by administrative staff, which have an effect on the overall operations of the unit. These include flow, stocking of rooms, scheduling tasks and all other duties as assigned. - Appropriately utilizes organizational resources including management of time, supplies, equipment and other assets. - Other duties as assigned. About the Role As a Right at Home Caregiver, you'll provide compassionate, in-home support that enables seniors and vulnerable adults to continue living safely and independently in the comfort of their homes. Your responsibilities will include: Providing personal care, including assistance with bathing, dressing, grooming, and toileting. Building meaningful relationships through companionship, conversation, and emotional support. Assisting with light housekeeping and meal preparation. Providing medication reminders. Assisting with mobility and transportation. Safety supervision and peace of mind. Following individual care plans and accurately documenting daily care notes using our mobile app. Communicating effectively with our office team. Skills & Experience We're looking for caring individuals who are passionate about helping others. You'll be successful in this role if you have: Successfully pass a background check. Reliable transportation and valid ID. Compassion, reliability, and a positive attitude. Excellent communication and interpersonal skills. Must be 18 years of age or older. Ability to read, write, and speak English. Previous caregiving experience, CNA, PCA, HHA, or companion care experience is welcomed but not essential.
