Job Summary

As a Registered Nurse MDS Coordinator (RN) (MDS), you will play a vital role in ensuring that our residents receive the highest level of care and attention. We are seeking a compassionate and experienced RN to join our interdisciplinary team, utilizing your expertise to improve patient outcomes and satisfaction.
Responsibilities
Coordinate and conduct Minimum Data Set (MDS) assessments for residents, ensuring accurate and timely documentation
Develop and implement individualized care plans, working closely with the healthcare team to achieve resident goals
Monitor and track resident progress, identifying areas for improvement and implementing necessary changes
Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care
Maintain accurate and up-to-date records, adhering to regulatory guidelines and company policies
Provide education and training to staff members, promoting best practices and continuous learning
Participate in quality improvement initiatives, contributing to the development of policies and procedures
Requirements
Registered Nurse (RN) license with current registration in the state of Nevada
Strong clinical skills and experience in long-term care, preferably in SNF settings
Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and responsibilities
Excellent communication and interpersonal skills, with the ability to work collaboratively with residents, families, and healthcare team members
Ability to maintain confidentiality and adhere to HIPAA guidelines
