Department Overview The special care nursery in Minneapolis is a 33-bed, 24-room unit and is part of the Mother Baby Center at Abbott Northwestern and Children’s Minnesota Minneapolis Hospital. The special care nursery team cares for babies who are 32 weeks and older and require a higher level of care than the newborn nursery provides, as well as full-term babies who may be sick, have an anomaly or are transitioning from the neonatal intensive care unit (NICU) to home. We care for these babies and their families and provide education to help them learn how to care for their baby at home. Our team of nurses, social workers, occupational therapists, physical therapists, neonatologists and nurse practitioners work together to provide a supportive and nurturing environment for the patients, families and staff. SUMMARY Position Type: Non-exempt, full-time or part-time, hourly role. Nurses at UVA Health conduct evidence-based practice within a structure that allows point-of-care clinicians to drive questions and solutions, with support from the larger structure of science, quality improvement and evidence-based practice. As a member of the Unit-Based Leadership (UBL) team, the clinical nurse leader (CNL) is responsible for supporting nursing professional practice through professional development, evidence-based practice, and unit-based quality improvement for one or more microsystem. The CNL trains, coaches and supports nurses and members of the interdisciplinary team to elevate professional practice in a manner consistent with organizational (UVA Health) standards, workflows, processes and strategic outcomes. Principal Responsibilities: This position reports to the Home Health Director Provides services by the plan of care. Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs. Initiates the plan of care and necessary revisions. Provides those services requiring substantial specialized nursing skills. Initiates appropriate preventive and rehabilitative nursing procedures. Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients promptly as per Agency policy. Coordinates services. Informs personnel of changes in the condition and needs of the patient. Counsels the patient and family/significant others in meeting nursing and related needs. Participates in and presents in-service programs. Understands and adheres to established Agency policies and procedures. Processes orders and notifies physicians of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries. Determines the amount and type of nursing needed by each patient. Refers to Physical Therapists, Speech Language Pathologists, Occupational therapists, and Medical Social Workers those patients requiring their specialized skills. Supervises and teaches other nursing personnel. Conducts patient care conferences on patients assigned to his/her care. Participates in peer review Quality Assurance and Performance Improvement as assigned. Gives total patient care as needed. Takes on-call duty nights, weekends, and holidays, as assigned. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharge, and significant changes in condition by agency defined time frames. OVERVIEW: Assist the Director of Nursing and Assistant Director of Nursing in planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the Nursing Department. Maintains quality clinical care for residents/patients in accordance with current rules, regulations, and guidelines that govern the Facility. RESPONSIBILITIES: • Supervises the clinical care planning and delivery processes to ensure the coordination of quality patient care. Reviews and updates the care plan accordingly • Assesses residents/clients upon admission and periodically as conditions change • Performs rounds to evaluate patient condition • Provides or assists with the delivery of direct patient care when indicated, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated • Obtains and implements Physician Orders, checking for accuracy and completeness • Communicates between shifts regarding changes in resident/patient conditions • Ensures a safe and secure environment for patients/residents, staff and visitors • Provides clinical assessment to MD/NPs on call for quality outcomes delivered to patients • Plans, implements and evaluates the clinical competencies and on-going educational/orientation needs of the staff. • Participate in Facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services • Participate in developing, implementing, and periodically updating the resident’s written discharge plan • Ensure that a sufficient number of nursing staff is available for each shift to ensure that quality care is maintained • Make daily rounds of the Nursing Services Department to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Report findings to the Director of Nursing Services • Review complaints and grievances made or filed by department personnel. Make appropriate reports to the Director of Nursing Services, as required or as may be necessary • Maintain nursing supplies inventory and other responsibilities as required • Assist in the Quality Assurance and Performance Improvement (QAPI) process QUALIFICATIONS: EDUCATION / EXPERIENCE: • Active RN/LPN Nursing License required • Current registration and license to practice as a Registered Nurse in the state employed in • Peripheral IV certification • Minimum 5+ years nursing experience in a hospital, nursing home or other healthcare setting and a minimum 1-year experience as a nurse supervisor • CPR/BLS certified • Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care centers. • The ability to read, write, speak and understand the English language required, knowledge of other language helpful SUPERVISORY RESPONSIBILITIES: • Supervises the nursing personnel and the day-to-day nursing activities of the facility during an assigned tour of duty WORK ENVIRONMENT: • This job operates in a health care setting. This role requires regular walking to various locations around the care center • Incumbent may be exposed to virus, disease and infection from patients and specimens in working environment • Incumbent may be required to work extended periods of time at a video display terminal Incumbent may experience traumatic situations, including psychiatric, and deceased patients • Works beyond normal working hours and on weekends and holidays when necessary • Must be able to stay or return to facility during emergency conditions PHYSICAL REQUIREMENTS: • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. • While performing the duties of this job, the employee is regularly required to talk and hear.
