Promote participant independence, dignity, and safety in line with PACE principles. Participate in IDT meetings, case discussions, and care planning. Fulfill responsibilities as part of the on-call rotation. Perform other duties as assigned to support participant care. Essential Requirements Current LPN license in the state of practice. At least 1 year of experience working with the elderly population (geriatric, long-term care, home health, assisted living, or PACE preferred). Current CPR/BLS certification. Valid driver’s license and reliable transportation for home visits. Strong communication and interpersonal skills to work effectively within an interdisciplinary team. Competence in basic computer skills and ability to use an EHR system. Flexibility to adapt to participant needs and program priorities. Primarily weekday schedule with participation in the on-call rotation.
