Summary: Provides personal care, light housekeeping and assistance with ADLs (Activities of Daily Living) as outlined in each SE participant’s plan of care in participant homes as well as at the PACE center; exercises independent judgement; reports any changes in participant status to the IDT; participates in carrying out infection control precautions and increased cleaning and disinfecting of the PACE center as assigned and in accordance with current CDC guidelines Responsibilities Under the supervision of the RN, assists with the Activities of Daily Living (ADL) needs of participants both at the PACE center and in participants’ homes (i.e., community, Assisted Living Facilities, Rest Homes, Supportive Housing programs, etc.) Contributes to the development of a care plan for participants through interaction with other members of the Interdisciplinary Team. Collaborates with members of the Interdisciplinary Team to assure appropriateness and continuity of care. Carries out non-skilled treatments including, but not limited to vital signs, transfers, toileting, bathing, dressing at the PACE Center and in the community. Assists the Supervisor, Recreational Activities and other activities staff with individual and group programs by helping to plan individual treatment programs, increasing participants’ motivation to participate, assisting participants to participate when needed, and assisting with evaluation of program effectiveness. Assists registered therapists and certified therapy assistants with treatments and participant-specific activities which are ordered for each participant and assigned by the registered therapist including, but not limited to, positioning, transfers, ambulation, and exercises. Maintains a clean and safe working and/or living environment in the PACE center and/or participants’ homes. Assists with meal and snack preparation, serving, feeding as needed, and clean-up. Uses safe techniques in all interactions with participants at the PACE Center and in participants’ homes. Provides accurate and timely documentation in the EMR and other systems as required by SE policies and procedures and/or as assigned by supervisor. Participates in carrying out schedule of daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines for increased precautions Actively participates in distribution of work for health aides to ensure care needs of participants and site tasks are completed timely and appropriately including but not limited to community-based care and center-based tasks, as assigned Actively participates in regular team meetings for health aides at respective site Participates in carrying out infection control precautions and increased cleaning and disinfecting of the PACE center as assigned and in accordance with current CDC guidelines Performs all duties in accordance with FH and Summit ElderCare policies and procedures. Qualifications Education: Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but we will accept equivalent work experience in lieu of certificate. Certification: Certification as a Home Health Aide or Nursing Assistant is desired. Reliable Transportation Required. Experience: One year of experience working with a frail and/or older adult population preferably in a PACE program. If under one year, must complete Summit Eldercare training program for health aides prior to working with participants independently. Must complete a standardized set of Summit Eldercare competencies for Health Aides on an annual basis. Pay Range Disclosure: In accordance with the Massachusetts Wage Transparency Act, the pay range for this position is $20.00 to $24.00 per hour , which reflects what we reasonably and in good faith expect to pay at the time of posting. SUMMARY: Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. The CLA’s responsibilities for each participant are specified in the CLA Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. The list should not be taken as an exhaustive list of all responsibilities, duties, and skills required of an individual assigned to this job. SPECIFIC DUTIES AND FUNCTIONS: The CLA plan of care is based on the initial and on-going assessments that are performed by the manager, assigned RNCM/clinical team, and lead CLA. Performs clerical duties for admissions, transfers, discharges, and deaths according to standards and facilitates patient throughput activities. Transcribes and processes orders according to unit standard. Releases and confirms ordered tests to appropriate departments. Coordinate's transportation of patients to multiple tests. Participates in obtaining and communicating electronic information as pertinent to the nursing unit. Schedules tests, faxes consults, etc. Assists in ordering supplies. Stationery items as well as floor stock, patient needs, individual nursing supplies. Communicates with food services to ensure appropriate patient diets. Operates, problem solves, and takes corrective measures for clerical equipment. To include scheduling maintenance of computers, fax machines, phones, etc. Staff have access to non-controlled medications storage and supply areas and can transport non-controlled medications as necessary to meet the needs of the clinic or unit. Assists with transporting non-controlled medications to and from Pharmacy and within the unit as needed. Assists with transporting blood products to and from Blood Bank as needed. SUMMARY: Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. The CLA’s responsibilities for each participant are specified in the CLA Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. The list should not be taken as an exhaustive list of all responsibilities, duties, and skills required of an individual assigned to this job. SPECIFIC DUTIES AND FUNCTIONS: The CLA plan of care is based on the initial and on-going assessments that are performed by the manager, assigned RNCM/clinical team, and lead CLA. The CLA assists in providing and/or maintaining optimal physical and emotional comforts to the participants (residents) living in the community setting while embracing the culture, mission, values, and the practicing Ten Principles at PACE SEMI. The CLA assists in creating a nurturing supportive environment while helping participants (residents) achieve maximum self-reliance and independence within the community living setting. The CLA performs the care or services that are outlined in the individual participant’s care plan; any care guidelines that are pertinent to the participant’s diagnosis are specified in the care plan. The CLA performs a variety of tasks and services for the participants (residents), including: Personal care up to and including 2-person assist 1:1 interaction in DHC and CL as outlined in the plan of care Assistance with activities Assistance with meal preparation / cooking/ feeding/other nutritional needs Assistance with toileting / managing incontinence Simple treatments Grocery shopping Laundry services Coordination of care with participant and the DHC based on care plan Engage with resident elevating health concerns and customer services issues or needs appropriately Homemaking services / chores/ housekeeping services/sanitizing equipment All other duties as required by manager and lead CLA, as assigned Rounds q2 hour after 7pm-7am. At this time the participant’s satisfaction with the Care Plan and CLA is evaluated Driving is required within PACE SEMI catchment area The CLA must have reliable transportation available on a daily basis, a current valid driver’s license, must maintain an acceptable driving record, and provide proof of current automobile insurance The CLA must have the ability to transfer and assist with moving patients; frequent walking, bending, and lifting of forty (40) pounds or more may be needed in the performance of duties The CLA may be required to change participant assignments based on the changing acuity level, participant needs, and participant census.
