Under the direction of the DNS, complete chart audits and report results and progress, and makes recommendations for improvement to the DNS. Ensure proper infection control techniques are utilized. Maintain effective communication with families, residents and community staff. Qualifications: Current RN license by State Board of Nursing in state employed; license must be active, valid, and in good standing. Experience in the long-term care or working with the geriatric population preferred. Must have at least three years nursing experience and one year supervisory experience. JOB RESPONSIBILITIES: Assessment: a. Initiate a nursing assessment of patient’s condition upon admission. b. Complete 4-week reviews in a timely manner and ensure all recommendations to the plan are activated in the provider orders. General: a. Function as an intake/discharge nurse during clinic times. b. Foster continuity of care between the Wound Care Department and caretaker (home, hospital, home health, etc.). c. Prepare clinic records to enable accurate and complete updates to the physician. d. Assist others within the clinic. e. Maintain open, professional communication with patients, physicians, and staff. f. Safely operate all clinical equipment. g. Utilize appropriate infection control procedures. h. Maintain skills and knowledge through training and continuing education. Complete mandatory education requirements.
