The Case Manager is responsible for a scope of care that encompasses all ages from infancy through geriatric and demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. The Case Manager collaborates with the provider to coordinate inter-disciplinary services which may include nursing, therapy, social work, home care aide, respiratory therapy, home medical equipment and home infusion services. The Case Manager is accountable for attaining the best possible patient outcomes including identification and focused oversight of patients at high risk for hospitalization.
