Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. Responsibilities What You’ll Do Collects comprehensive patient health data/history including past and present medical, surgical, family, and social history. Diagnostic tests and procedures relevant to the patient’s status are collected, interpreted, and documented at the Electronic Medical Record (EMR). Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management. In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis. In addition, develops a comprehensive plan of care that includes interventions of treatments to attain outcomes. Evaluates patient progress in attaining expected treatment outcomes. Monitors for signs and symptoms related to disease progression/recurrence. Appropriately manages medical and anticoagulation management emergencies according to protocol/policy. Prescribes and/or orders medications. Obtains appropriate consents for surgery and procedures. Ensures completion of all needed pre-operative tests, forms, and documents. Documents assessments, prescribing, plan, modifications to plan, and evaluation of care in the EMR. Qualifications What You’ll Bring Required: Completion of an educational program which is designed and conducted to prepare graduates to practice as an NP/APN with a master’s degree in nursing and eligible for certification by a national certifying body. A current Registered/Professional Nurse license and registration in the State of New Jersey is required. Complete all in basket requests assigned to individual team (as well as covering for other nurses who may be temporarily absent), including responding to patient messages, prescription refills, lab results, referrals, home health agencies, medication prior authorizations, patient forms and follow up from physicians. All tasks must be reviewed, addressed and marked appropriately before the close of each business day. Respond to questions from patients, family members, faculty/residents, other health care providers in a timely and professional manner. Patient phone calls should be answered on the same day they are logged in, before leaving for the day. If patient is not home at the first attempt, a notation should be made in the medical record. All patient conversations must be documented in the electronic medical record. Takes and records patients’ vital signs Administers specified medication, orally or by subcutaneous or intermuscular injection, and notes time and amount on patients’ charts. Overview Responsible for front and back office procedures supporting the Maternal Fetal Medicine division located at the Department of Ob/Gyn Princess Anne office. Also assists Physicians and Nurses with medical examination of patients. Responsibilities Answers incoming telephone calls. Triage and refers calls as necessary. Also responsible for taking messages, paging doctors when appropriate, handling hospital consults, and answering inquiries. Schedules appointments with doctors, calls patients to remind and confirm appointments; reschedules patients from the bump list, and schedules residents’ clinics where appropriate. Prepares charts for the next day; creates new patient charts, files and pulls charts and checks charts for referrals and authorization prior to visit. Also prepares charge tickets. Greets patients in a courteous manner, marks off names as patients arrive, and notifies nurses/medical assistants/MDs of arrivals. Registers old and new patients, verifies demographics and insurance, determines insurance copays, and collects money and copays to send to EVMS Medical Group. Qualifications Successful completion of a Medical Assistant program. Equivalent military training will be accepted. Must have certification (CMA or RMA) or ability to obtain within 90-days of employment. Completion of BLS class and re-certify every 2 years. Previous medical office experience with relevant functions in a doctor’s office preferred. Must have some working knowledge of computers. Customer service-oriented individual is required.
