Senior Registered Nurse - Hospital at Home

Location: Acute Care Hospital Tower Shift- 12hr Days (7am-7pm) Work Hours: This program is a 7 day a week service, rotating every other weekend and rotating holidays Employment Type: Part Time with benefits *must be able to work a minimum of 24 hours per week* **This position will be onsite and candidates must have the ability to travel within a 20 mile radius approximately 50% of the time Hospital at Home is an innovative program that combines in-home with remote monitoring, offering patients timely, hospital-level care in their homes. Though the list of diagnoses enrolled in Hospital at Home continues to expand, common disease states include acute pneumonia, cellulitis, COVID-19, urinary tract infection, dehydration, or exacerbations of chronic conditions, such as heart failure or chronic obstructive pulmonary disease. For many patients, the program provides an alternative to the traditional hospital setting and results in improved patient outcomes and satisfaction. The Hospital at Home team is looking for motivated nurses with critical thinking skills, acute care experience, and an interest in serving the Parkland community. Primary Purpose The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate acute patient care delivery in the patient home. Using the nursing process, the RN assesses the patient in their environment, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN utilizes knowledge of patient needs and providing healthcare in the home environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, each patient's condition, and the competencies of the employee. Minimum Specifications Education Must be a graduate from an accredited school of nursing. Prefer Bachelor of Science in Nursing. Experience Must have two years of nursing experience in the acute care setting. Prefer experience in the home environment. Equivalent Education and/or Experience None Certification/Registration/Licensure Must have current, valid RN license or temporary RN license from the Texas Board of Nursing; or, valid Compact RN license. Provides patient care educational information and training to patient and family members. May serve as a resource for specific clinical patient care issues. Acts autonomously to structure day as well as coordinate and deliver care to multiple patients in Dallas County, following expectations outlined in department nursing procedures. Completes appropriate patient assessments, admission criteria, identification and implementation of appropriate clinical interventions, discharge planning, interdisciplinary communication. Utilizes critical thinking skills and recognizes emergency situations and provides appropriate nursing interventions. Consults with other team members as required, notifies providers to report changes in patient condition and implements orders in a timely manner. Exercises independent judgment in providing safe nursing care to patients. Completes appropriate documentation by maintaining accurate and timely entries and notations in patient care records and hospital documents. Ensures that all records and documents meet the standards, goals, and objectives of the department and Parkland. Maintains and implements knowledge of applicable rules, regulations, policies, laws and guidelines that impact or govern Radiology nursing practice, and ensures that Parkland Hospital is in compliance. This may include EMTALA, Order of Productive Custody, Memorandum of Transfer and APOWW. Develops effective internal controls that promote adherence to applicable state/federal laws and the program requirements of accreditation agencies and federal, state, and private health plans. Maintains and updates knowledge of sanitation, infection control, safety, supplies, equipment, appropriate utilization of supplies and materials. Position Summary: Accountable to provide clinical navigation for identified patients within the Penn Medicine LG Health system. Performs clinical and administrative functions to support the coordination of inpatient and or outpatient care needs. Patient care includes care to patients in the following age groups: adult 19 through 64 years , and geriatric 65 years and above . Manage physical, mental, psychological and or spiritual well-being while working with the patient to develop a treatment plan and contribute to its implementation and evaluation. Provide continuity of care throughout the entire experience. Remain a point of contact for the patient along the continuum of care: Respond appropriately to instances of unsafe practice to safeguard the patient’s health; Maintain an environment conducive to health and healing including infection control; Provides referrals to supportive resources at local, regional and national levels; Provides treatment summaries and care plans for patients after receiving treatment. Provides education to patient caregiver on items such as: disease symptom management, medication purpose and side effects, diet, rehabilitation and when to notify the healthcare provider. Assess the health needs of the patient using a bio-psycho-social-spiritual framework including self-determined health goals and develop a plan of care for the patient based on his or her own personal needs. Participates in related national and local professional nursing organizations, reviews literature and assess trends in patient care needs and development of services and resources to meet these needs. Performs nursing techniques proficiently by maintaining accurate documentation, using appropriate channels of referral and working effectively with other health care team members. Assessment of patient’s ability to navigate through the healthcare system and identify potential barriers. Guide patient through the healthcare system including: Appropriate referrals to nutrition support, social work, financial counseling, psychology, rehab and or palliative care. Ability to communicate among specialty practices with the healthcare organization- build relationship with physicians and staff. Screen and inform patients who may be eligible for clinical trials for advancement of practice. Advocate and facilitate access to the appropriate services to individuals with disabilities and advocate for equal access for all patients regardless of financial status. Advocate for the patient’s right to autonomy and an informed decision-making process. Identify and implement methodologies for continuous quality improvement within the service line and support disease specific performance improvement initiatives. Actively engage and participate in community outreach, education and supportive services programs, including appropriate screening programs or educational and health awareness promotion activities. Minimum Required Qualifications: Current licensure as a Registered Professional Nurse, issued by the PA State Board of Nursing. Bachelor’s degree in nursing, healthcare management or related field. Certification in related specialty required within 3 years of hire; Oncology Certification issued by OCN, OBPN or CBCN Three 3 or more years of experience as a clinical nurse in related specialty. CPR Certification - American Heart Association AHA Basic Life Support, formerly Healthcare Provider AHA Heart Saver AMA CPR BLS certification required within 30 days of hire. A health care provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e. not excluded from participation in Medicare, Medicaid or any other federal or state health insurance program. Select and recommend collection devices/undergarments. Maintain data and reports and requested. Participates in quality improvement. Develop practice standards for managing patients with wound ostomy and incontinence care needs. Monitor patient outcomes on an ongoing basis. Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
