Job Description The PACE Center Manager RN has full responsibility for all disciplines and services provided through Centra PACE Center and administrative tasks related to direct operation of the program. This role provides clinical oversight to the Lynchburg PACE program, Bedford Alternative Care site, and Amherst Alternative Care site. Responsibilities Essential Duties and Responsibilities: Manages the daily operations of the Lynchburg PACE site under the guidance of the Site Director Coordinates care planning and implementation, preparation for interdisciplinary conferences, and meetings with family and participants. Serves as liaison between individual program disciplines, other PACE departments, and community services providers. Communicates on a daily basis to ensure the information is being conveyed to the IDT for: (1) care planning, (2) morning meetings, (3) documentation into the PACE participant’s medical record for services received at the site. Participates in the PACE leadership team and serves as liaison between administration and direct care staff. Participates in the development of the Interdisciplinary Team (IDT) Plan of Care. Ensure compliance with local, state and federal regulatory requirements. Ensure care plans are accurate and in compliance with all regulatory requirements. Manage employee performance which includes regular evaluations, mediating disputes, and administering progressive discipline. Collaborates with direct reports to create, operationalize, and monitor action plans developed as a result of feedback from Employee Engagement Survey and other surveys. Follows-up on risk management concerns within specified time frames. Holds direct reports accountable for Centra safety behaviors. Operates Site within computed productivity targets. Operates Site within budget. Create schedules that meet core staffing needs or develops staffing action plans to address deficiencies. Oversee and holds Site accountable for appropriate utilization and charging of center inventory. Liaisons with Human Resources on complex issues such as Family Medical Leave Act (FMLA), Worker’s Compensation, recruitment, disciplinary actions, employee engagement, etc. Collaborates with direct reports to create, operationalize, and monitor action plans developed as a result of participant satisfaction survey. Responsible for Clinical oversight in the Lynchburg PACE center, Amherst Alternative Care Site, and Bedford Alternative Care site. The role provides direction to PACE Clinical Supervisors at Amherst, Bedford, and Lynchburg. Direct reports include three PACE clinic supervisors, Physical therapy, Occupational therapy, Dietician, and masters social workers. Directly reports to the Lynchburg Site Director Other Functions Plans, coordinates, manages, and documents the care provided to participants. Participates in the Program’s Quality Improvement Plan. Displays and promotes a positive public relations approach to staff, physicians, participants and their families, and the community. Ensures compliance with departmental policies and procedures. Identifies and resolves customer service issues. Presents PACE-related information throughout Centra and the community. Contributes to program development through building relationships with potential referral sources. Develop formal or informal mentoring relationships. Handles conflict resolution in an appropriate, timely and consistent manner. Ensures that staff operationalize Centra's best practices. Applies process improvement methodology to center-based initiatives. Follows-up on risk management concerns within specified time frames. Coordinates all activities relating to new enrollees. Performs other duties as assigned. The Regional Director of Clinical Reimbursement provides extensive training, analysis, advice and consultation to the facilities and teams within his/her area of responsibility. Ensures compliance with federal and state regulations, as well as Company policy and procedures regarding state case mix/Medicare and Managed Care payment systems. Monitors, consults, and makes effective recommendations for changes and modifications to existing facility processes, systems, policies, and practices which will assure efficient, effective and compliant state Medicaid/Medicare/Managed Care payment performance. Studies, analyze and reports period over period information and systems in order to identify trends and deviations from results in Medicare, Managed Care and State Case Mix Index and takes appropriate actions. Works in conjunction with teams to resolve issues effecting deviations from expected results. Recommends changes and performs follow-up to ensure that those recommendations are effectively implemented and monitored for appropriateness. Regularly communicates to management outside the facility on recommendations made to facility management to ensure proper implementation and follow-up.
