Participate in IDT meetings, family conferences, and case reviews; contribute to the development and adjustment of Plans of Care. Complete timely, accurate documentation in the medical record; meet all agency and regulatory charting, reporting, certification/recertification, and discharge summary requirements. Support utilization review, peer review, and quality management activities as assigned. Setting-Specific Responsibilities: Clinic (Center-Based): Prepare, stock, and maintain exam and triage rooms; ensure availability of required supplies and materials. Assist with participant examinations, testing, and treatments performed in the clinic. In the absence of the Nursing Services Director/Clinic Nurse Supervisor, supervise clinic staff and support overall clinic workflow. In-Home Services: Provide skilled nursing care in the participant’s place of residence; deliver preventive, restorative, and rehabilitative nursing procedures. Determine type and amount of nursing care needed for each participant and coordinate with Home Care resources. Supervise LPNs at least every 30 days and HHAs/CNAs at least every 14 days (or per regulatory requirements); provide teaching and competency reinforcement. Supervises unskilled care every 90 days. Conduct supervisory visits, care conferences, and periodic participant/caregiver check-ins; trend data and collaborate with the Home Care Manager and Nursing Services Director to ensure quality care delivery. Safety, Compliance & Operations Adhere to InnovAge policies and all applicable federal, state, and local regulations (e.g., HIPAA, OSHA, infection prevention, medication safety, scope of practice). Use PPE and follow standard, transmission-based, and universal precautions. Participate in on-call, after-hours, and holiday coverage as assigned. Job Description: Essential Duties & Responsibilities including but not limited to: • Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home • Navigating patients and families across the continuum of care • Monitoring and responding to biometric data per Hospital at Home Command Center protocols • Conducting audio/visual patient care visits as directed by the care team and/or protocol • Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approach • Leveraging the resources and expertise of supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation • Adhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care • Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts Clinical Roles and Responsibilities: • Providing protocol driven assessment, care coordination and interventions as appropriate informed by Hospital at Home protocols for diverse patient populations with a focus on oncology, medicine and post- operative care • Exhibiting sound clinical decision making and critical thinking skills • Maintain and practice in accordance with nursing guidelines per state and hospital regulations • Facilitating admissions to l Hospital at Home utilizing Hospital at Home guidelines • Serving as a role model for professional nursing practice • Utilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems • Sharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise • Supporting research efforts to advance knowledge and promote evidence-based practice • Preforming other duties and responsibilities as assigned by the Senior Director of Nursing Quality and Safety Responsibilities: • Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents • Participate in quality and safety investigations and root cause analyses • Take appropriate action to ensure patient safety as a result of any investigation; and • Serve as a resource in quality improvement processes and technique Organizational Requirements: 1.
