Full Time Registered Nurse Case Manager, Level 2 Anticipated Territory: Woodland and surrounding areas What you’ll do as a Registered Nurse (RN), Level 2 Home Health Case Manager: Utilize your outstanding clinical skills as you create, implement and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes. In this leadership role, you will have oversight of 2 Nurses who are responsible for executing your care plan.
