Responsibilities : Responsible for assisting patients with and per plan of care, the following: Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency) Safe transfer and ambulation techniques Exercise according to the plan of care Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency) Patient’s laundry Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.) Observation and teaching of home safety practices Recognition of an emergency situation and knowledge of emergency management procedures Must follow patients’ plan of care and complete all tasks assigned Communicates in a constant, clear, and effective way with the client, family/caregiver and office Observes and documents patient condition and reports any changes, to the assigned nurse supervisor Requirements : Meets all training requirements in accordance with state and federal laws when applicable Ability to read, follow written instructions and document care given Possess and maintains current CPR certification in accordance with state regulation PPD or chest x-ray if applicable Successful completion of Maxim screening examinations Participation and completion of Maxim’s competency program Must meet all federal, state, and local requirements The Lakes benefits : part-time position. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. What is Required? Current, unrestricted state RN licensee 1+ year experience in a clinical care setting as a nurse Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Able and willing to travel within branch/office coverage area. Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. What you’ll do: Assist clients with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting Monitor and record vital signs (BP, pulse, temperature, respiration) Support mobility, transfers, and fall prevention. Assist with meal preparation and feeding. Perform light housekeeping tasks. Provide companionship and emotional support. Observe and report any changes in the client's condition to the care team. Follow individualized care plans and agency standards. Qualifications: Valid CNA Certifications Current CPR certification (required) Minimum 1 year of caregiving experience (in-home care preferred) Valid Driver's License with a clean driving record.
