What You Will Do to Change Lives The LPN provides recovery focused services related to the safe and appropriate administration of medical treatment (including medications) as prescribed by the physician. Provides safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of care. Shifts Available: Full Time, 32 hours a week, Days/Shifts Vary as Needed - 3:00pm - 11:30pm Expected starting wage range for LPN is $23.64 - $29.22 (hourly) Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements. Depending on what shifts you are asked to work: Shift differentials for hourly staff (6% for PM Shift, 10% for Overnight Shift). Weekend Shift differentials for hourly staff (5% for Weekend AM Shift, 11% for Weekend PM Shift, 15% for Weekend Overnight Shift). ESSENTIAL FUNCTIONS • Demonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholders. • Establishes and maintains a therapeutic relationship with members served • Provides nursing care and assists in the welcoming, admission, and discharge process • Collects data relevant to members served care; develops and implements nursing care plans • Demonstrates knowledge of crisis management and intervention • Transcribes physician orders accurately and in a timely manner • Administers medication and treatments to members served • Receives and presents change of shift reports as assigned • Adheres to medication management policies and procedures • Notifies the prescriber and nursing supervisor of any changes in behavior or condition • Documents the progress of members served toward their Recovery Plan goals • Attends and participates in community meetings and groups • Actively participates in multidisciplinary team meetings and treatment planning meetings • Coordinates and participates in counting medications and giving report at the beginning and end of each shift • Must attend CPI training. Must assist with de-escalation as needed. Duties and responsibilities may be added, deleted, and/or changed at the discretion of management. What You Bring to the Table (Must Have) • Graduation from an accredited LPN program and current LPN licensure in state of operation • Current CPR/BLS certificate • Must be at least 18 years of age • Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment • All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment. Follows up on tests, lab results, thoroughly and within established time frames. Lab tests are followed up on within 48 hours after test results obtained. Considers cultural, cognitive, emotional, and social influences on the patient/families ability and readiness to learn. All teaching is documented on the FER to include understanding of topics taught. Provider Support Gathers patient information in a timely, concise, thorough manner and conveys this to the MD. Assists MD in clinic to maintain a good workflow and meet expectations of customers in relation to being on time. Demonstrates effective delegation techniques. Performs special tasks, procedures within the scope of practice for an RN that is unique to Ambulatory Care clinics i.e. (database entry, scheduling of special procedures, as well as assisting in special procedures, inpatient role, case management, and education/teaching). Completes with expertise, timely, organized and with detail. Communicates effectively through written documentation or verbally to appropriate individuals. Coordinates patient care to ensure continuity of care This role will require weekend call from 8am- 4:30pm for inpatient diabetes education. Establishes integrated governance structures, standards, and processes that promote nursing excellence, consistency, and shared accountability across the enterprise. Advances professional governance, strengthens nursing voice and leadership, and embeds Magnet principles into clinical professional practice. Integrates nursing informatics/innovation into professional practice standards, governance structures, and clinical workflows to promote consistency, evidence-based decision-making, and nursing engagement across the enterprise. Clinical Education, Workforce Development & Academic Partnerships Oversees clinical education (i.e. nursing, RT, support) and workforce development strategies, including transition-to-practice, competency-based orientation, simulation, resuscitation, continuing education, and learning technologies. Ensures nursing informatics competencies are embedded across orientation, continuing education, simulation, and transition-to-practice programs, preparing the workforce to effectively use digital tools, clinical decision support, and data-driven practice models. Leverages simulation as a core education modality. Ensures compliance with accreditation and licensure standards while driving adaptive change in response to evolving care models and regulatory requirements. Serves as the clinical liaison to academic partners and schools of nursing, strengthening academic-practice partnerships that prepare nurses for current and emerging roles across the continuum of care. Evidence-Based Practice, Research & Clinical Inquiry Leads enterprise systems and infrastructure supporting evidence-based practice, nursing research, and clinical inquiry. Education Qualifications Doctoral degree is required.(DNP or PhD in nursing is preferred). Graduate from an accredited school/college of nursing required. Bachelors preferred. Licensure : Registered Nursing licensed in the State of Nebraska or eligible compact state required. Certification : Required: Basic Life Support (BLS), Pediatric Advanced Life Support (PALS) and Advanced Cardiac Life Support (ACLS) required or completed by end of orientation. Preferred: Certification in Emergency Nursing (CEN), Critical Care Nursing (CCRN), Trauma Nursing Core Course (TNCC), and/or Emergency Nursing Pediatric Course (ENPC) Experience : 1+ years of higher acuity RN Experience required. Required Knowledge/Skills/Abilities Ability to rapidly assess patients and prioritize treatment based on severity of condition and provide life-saving interventions when needed Critical thinking skills used to make quick decisions and adapt treatment plans based on changing patient conditions Effective communication and collaboration with patients and their family, team members, and other healthcare professionals to ensure coordinated patient care Capability to work effectively under pressure while maintaining a compassionate approach to patient care Perform technical skills competently and independently to assure patient safety Detailed oriented Essential Job Functions Formulate clinical decisions and implement multi-dimensional plans of care that demonstrate independent judgment and critical thinking consistent with private, state, and federal regulations. Documentation reflects accurate assessment/evaluation, modification, and patient's response to therapy, changes in condition, and therapeutic interventions/procedures. Promote and enhance collaborative practice within the health care team to meet the physical and psychosocial needs of patients. Demonstrate continued professional growth by participating in opportunities to assure continued adherence to professional standards and provision of quality patient care. Physical requirements: This position is classified as Medium Work in the Dictionary of Occupational Titles, requiring the exertion of 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work. Must be able to pass background check. We also conduct pre-employment physical and drug testing.
