Completion of CHHA program. 1 to 2 years direct patient care experience in an institution or home care setting. Appropriate understanding of patient care requirements within certification. BLS required. Ability to communicate effectively verbally and in writing. Able to manage time effectively. Bi-Lingual is a plus. Current California driver's license and proof of insurance. Hospice Admissions RN (weekends)

The Admissions RN is a registered nurse and an Interdisciplinary Team member whose primary responsibility is to present ANCHOR Hospice services and philosophy to patients and families in a manner that results in the admission of appropriate patients. Participation in the information gathering, analysis, education and form completion aspects of the admissions process Evaluate patients for hospice appropriateness, e.g. medical records review as requested. Obtain agreement for admission of referred patients and families who meet admission criteria. Analyze data collected by the LPN to facilitate/obtain admission orders Build effective referral source relationships to support the high quality and responsiveness expectations associated with ANCHOR HOSPICE. The Patient Care Coordinator maintains a comprehensive working knowledge to assist in training new clinical staff, and to serve as a resource for field staff. Essential Job Functions/Responsibilities: Coordinates training for new hires; to include EMR training, orientation books, and clinical supplies. Assists Quality assurance with chart audits, clinical authorizations review and auditing compliancy for patients. Works with the Director of Patient Care Services and staff clinicians to gather, collate, and report patient care summary information to Case Managers. Regularly participate in interdisciplinary group meetings and ensure through attendance and record reviews that meetings are conducted in accordance with Agency and CMS regulations. Maintains comprehensive working knowledge of patients and ensures the needed documents are received and organized in the system. Works with all members of the clinical team to ensure that they are following the physician plan of care orders.
