JOB DESCRIPTION SUMMARY - LPN The licensed practical nurse provides hospice care and is experienced in nursing, with emphasis on community health education/experience. The practical nurse works with the registered nurse in providing services to meet the needs of individuals and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes a routine nursing assessment of patients and families to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides nursing care by utilizing all elements of the nursing process. 3. Assesses and evaluates patient’s status by regularly re-evaluating patient and family/caregiver needs. 4. Follows the plan of care and communicates necessary revisions needed as patient status and needs change. 5. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 6. Counsels the patient and family in meeting nursing and related needs. 7. Provides health care instructions to the patient as appropriate per assessment and plan. 8. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 9. Collaborates effectively with RN Case Manager and Clinical Supervisor and assists in coordinating patient care for assigned patients. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons in relation to the care plan. 4. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 5. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the case manager and interdisciplinary group. 6. Provides and maintains a safe environment for the patient. 7. Assists the patient and family/caregiver and other team members in providing continuity of care. 8. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 9. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing. 2. LPN, with a License in good standing to practice. 3. The LPN shall have a minimum of (a) two (2) years of experience as a nurse within the last three (3) years; OR have a minimum of one (1) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills. 7. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Provides appropriate support at time of death and perform bereavement assessment. What is Required? Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Current State License as a Registered Nurse RN 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager. Be the Best Hospice Aide You Can Be If you meet these qualifications, we would love to meet you: Certified by the state as a nurse’s aide, with completion of required training and a passing score of 85% or higher on the agency’s competency exam; additional requirements vary by state (e.g. CHHA in CA/NJ, CGNA in MD). CPR certification required for direct patient care in Georgia and Washington. Must be listed on the state aide registry with a clear record and eligible to provide care in homes, nursing facilities, or hospitals. Requires a reliable vehicle, valid driver’s license, and state-required auto insurance. Responsibilities: Provide compassionate, hands-on care to hospice patients, including assistance with personal hygiene, repositioning, and mobility support for individuals who may require lifting (up to 200 lbs with assistance). Support patients in their homes or care facilities through frequent movement and physical activity, such as walking, standing, reaching, and occasional driving, ensuring timely and dignified end-of-life care. Monitor comfort levels, respond to needs, and assist with daily living activities, always maintaining patient safety and dignity. Work comfortably in home and facility environments, adapting to varying conditions such as temperature changes, moderate noise, and standard healthcare exposures, while upholding infection control and safety protocols.
