We are seeking a dedicated Hospice Nurse Practitioner to perform Face-to-Face (F2F) encounters essential for hospice eligibility certification and recertification. In this role, you will gather comprehensive clinical information, perform detailed assessments, and collaborate closely with hospice physicians and interdisciplinary teams to ensure quality patient care and regulatory compliance. Key Responsibilities: Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes. Document clinical findings accurately and attest F2F encounters in patient medical records. Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements. Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3.
