Consult program medical director as needed; comply with applicable laws and regulations with respect to Collaborative agreements. Initiate reimbursement for services rendered. Assist in development of clinical practice guidelines/standards/protocols in support of quality palliative care. Provide training and continuing education for staff on palliative care. Meet monthly to provide reports and outcomes in relation to Palliative Care team goals and objectives. The leadership role of the Practitioner includes patient and family expectations; group dynamics to affect positive change; and provides a strong image as a role model. The practitioner will also supervise all RNs who are assisting with the program. INTERPERSONAL RELATIONS Communicates and collaborates with physicians, nurses and other multidisciplinary team members involved in the care of patients. Acts as patient advocate constantly monitoring, tracking, assessing and evaluating quality of care Promotes communication with referring physicians and the entire team regarding plan of care Promotes professional development of the team. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face to face assessments and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team. The nurse practitioner builds from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities. Essential Job Functions/Responsibilities Patient Care 1. Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. 2. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record. 3. Manages health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and / or treatments, obtaining consultations or making referrals and coordinating hospice care services. 4. Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification. 5. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. 6. Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. 7. Assures that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members. 1. Reviews and develops protocols for treatment and proposes options for interventions based on the literature in collaboration with the collaborating physician. 2. Consults the collaborating physician when the patient's plan of care is outside standardized practice and protocols. Communication 1. Communicates and collaborates with the interdisciplinary group to create, review and revise the patient's plan of care. 2. Consults with the patient's attending physician, the hospice medical director and other healthcare practitioners regarding the patient's eligibility for hospice care, ongoing care needs and medical management related to the patient's terminal condition. 3. Communicates with other community health practitioners to coordinate the plan of care. 4. Attends and participates in hospice interdisciplinary group meetings. 5. Completes, maintains and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record. 6. Educates/counsels patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to identify the goals of care and meet the care needs of the patient and family/caregiver. 1 Serves as a nursing resource for consultation and educations to members of the interdisciplinary team and other healthcare practitioners in the community. 1 Establishes, builds and nurtures relationships with staff and community referral sources to facilitate program growth. Additional Duties 1. Maintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services 2. Ensures compliance with the Medicare conditions of participation and other state regulations govern the provision of healthcare. 3. Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies. 1. Participates in organizational monitoring of the quality of medical services and quality improvement initiatives. 2. Assumes responsibility for personal growth. Develops, maintains and upgrades professional knowledge and practice skills through attendance at seminars, conferences and participation in continuing education and in-service classes. 3. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of chapters 246- 335 WAC and 70.127 RCW to the Washington Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of RCW 74.34.020 or 26.44.030 to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 3. Due to nature of job and working with multiples facilities with multiple covid guidelines must be covid vaccinated. 4. High school diploma or General Equivalency Degree (GED) 5. Ability to read and follow written instructions and document care given. 6. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. 7. Prior experience in hospice, orderly, acute , rehabilitative, or long term care facility. 1 year previous home health/hospice experience preferred 8.
