Qualifications Other information: Knowledge Current State of Illinois Registered Nurse licensure required. Maintain current BLS certification. Bachelor’s degree in Nursing (BSN) or Associate’s degree in nursing (ADN) (New graduates must have acceptance into an accredited school of nursing within 6 months from date of hire, Within 1 year from the date of hire, employee must be enrolled in a BSN program. RNs hired prior to 10/22/2020 are grandfathered in with an Associate Degree in Nursing (ADN) or diploma in nursing, with no further need to obtain a Bachelor’s degree in Nursing (BAN or BSN). Chief Nursing Officer must provide approval for any RNs with ADN degree hired after 10/22/2020, based upon unit need. Position Summary The Transplant Nurse Coordinator (CNCII) is a Registered Nurse with experience in and knowledge of transplantation and living solid organ donation. Practicing under the supervision of transplant surgeons and physicians, the transplant Nurse Coordinator ensures coordination of the clinical aspects of transplant patient and living donor care throughout the pre-transplant phase. The job duties vary with the particular setting or assignment. Transplant Nurse Coordinators can be assigned to kidney, pancreas, liver and/or small bowel programs. Clinical Nurse Consultant II is accountable for facilitating the delivery of patient care utilizing the Nursing Process and Relationship-Based Care model as the framework. The Clinical Nurse Consultant II provides patient care and consultation, coordinates the educational development of staff members and acts as a role model in clinical practice. The CNCII strives to improve patient outcomes by integrating Shared Leadership, Evidence Based Practice Duties & Responsibilities • Evaluates patients at the appropriate UI Health site of care. • Coordinates care for transplant recipients from referral and /or until waitlisted, all potential recipients with living donors and all living donor patients. • Ensures that all pre-transplant patient coordination and patient education is documented and is in compliance with UNOS, CMS transplant program guidelines and UI Health policies/procedures. • In collaboration with the provider, orders all testing for evaluation as needed and reviews results with the provider in a timely manner following UNOS and CMS guidelines. • Develops, coordinates, and communicates the patient's plan of care based on ongoing assessment of the patient and input from the patient's family. • Uses clinical expertise to collaborate with multi-disciplinary health care team to improve patient outcomes. • Consults with and implements orders by performing interventions as prescribed by the provider and documenting accurately in the electronic medical record. • Assesses, engages and obtains clinical information from multi-disciplinary team, patient and/or family to develop an appropriate patient educational plan. • Recognizes symptoms, establishes priorities and responds to patients changing needs throughout the transplant care continuum. • Ensures that the patient is added to the UNOS waiting list and the required listing letters are sent in a timely manner per UNOS requirements. • Coordinates and schedules all living donor transplant surgeries as assigned. • Attends and participates in transplant QAPI meetings, listing meetings, department meetings, multidisciplinary planning meetings and other meetings as needed. • Provides cross coverage for the transplant data coordinator by entering living donor information in UNOS prior to the living donor surgery as assigned. • Engages nursing and support personnel by delegating appropriate tasks per the Illinois State Nurse Practice Act and provides, support and feedback • Demonstrates a commitment to UI Health's values through teamwork and effective communication in an effort to achieve quality outcomes, patient safety, customer satisfaction and a clean, safe and therapeutic environment. • Integrates theoretical knowledge, evidence-based research, current standards of practice and clinical experience to provide “quality" patient care as measured by established nursing quality standards and other governing/accrediting health care bodies. • Recognizes and supports Shared Leadership as the selected method of nursing governance. • Participates in organizational, departmental and shared leadership activities; supports department and unit goals in daily behavior. • Collaborates with other nurses and physicians to identify, promote and provide positive learning experiences for staff and students. •Initiates, conducts and analyzes evidence-based practice research to improve patient care outcomes. Job Description The Registered Nurse will: Provide professional nursing care for the comfort and well-being of patients, which includes performing specialized treatments, administering prescribed medications, and coordinating patient care with other departments. Collect pertinent physiological, psychosocial, and functional data to formulate nursing diagnosis using evidence-based techniques and instruments. Assist physician during examinations and treatments. Formulate nursing diagnoses by interpreting data from health team members and integrate into written assessments. Assess patients, identify changes in status, establish priorities for patient care, and identify crisis situations when needed. Implement interventions consistent with the established plan of care and interdisciplinary treatment plan, evaluate progress toward goal attainment, and modify the plan of care as indicated. Provide information to patient and significant others regarding care during hospitalization, expected outcomes, home care instructions, and referrals to appropriate health agencies and resources. Prepare and provide reports on patient care plans to health team members, which includes completing assignment sheets, providing explanation of patient care plans, and completing documentation. Collect and report pertinent physiological, psychological, and functional data based on patient plan of care or patient need. Review, evaluate and make recommendations regarding SRAlab policies and procedures related to patient care to ensure comprehensive patient care. Perform all other duties that may be assigned in the best interest of SRAlab. Reporting Relationships: Reports directly to the Nurse Manager. Knowledge, Skills & Abilities Required: Professional knowledge of practice acquired by obtaining a Bachelor/Master of Science in Nursing from an accredited program or accredited Associate Degree in Nursing (with commitment to complete BSN/MSN within 2 years). Illinois license to practice as a registered nurse. Current CPR or BLS certification from the American Heart Association or American Red Cross required. Understanding of the growth and development of infant, child, adolescent, young adult, middle-aged adult and geriatric patients. Demonstrate the ability to interpret growth and development related information to assure patient needs are met. Ability to communicate effectively using timely verbal, non-verbal and written communication. Ability to take ownership for serving the customer, team and organization, for achieving goals and for resolving problems. Willingness to adapt to changing work demands and work environment. Demonstrated critical thinking skills and ability to make sound judgments to monitor and respond to patient needs or changes in condition. Ability to anticipate, assess and address the needs of patients Ability to transfer and position patients, consistent with lifting up to 35 lbs. Working Conditions: Normal patient care environment with little or no exposure to dust or extreme temperature. Exposure to moderately adverse working conditions in performing certain patient care activities. Occasional exposure to infectious diseases when working with patients.
