Responsibilities Serves as a resource and liaison to the Transfer Center Specialists that coordinate transfers and acceptance of patients for USA Health hospitals Collects clinical information for a transfer request and facilitates the transfer by matching the patient's needs to the facility with bed capacity and service capability Works in collaboration with multiple departments to foster relationships with referring facilities, physicians, and hospital staff Works closely with the Transfer Center team and leadership to identify trends and report barriers to patient throughput Performs transfer call center functions in a responsive manner answering calls, entering into a database consisting of facility service profiles, approved clinical protocols, established patient care protocols, and healthcare information Coordinates transfer of emergent, urgent and elective patients to a final destination facility from all potential referral sources to include but not limited to hospitals, clinics, nursing homes, and physician offices via a single call to initiate acceptance and expedite decision making relative to a potential transfer Facilitates patient throughput to a final destination with an ongoing focus on quality and efficiency Works directly with Transfer Center Specialists to ensure timely transfer, appropriate bed assignment, and provides accurate patient information to assist with the development of an initial plan of care Ensures accurate and concurrent data collection and maintenance Process patient acuity and facilitate patient flow through the continuum of care Triages and prioritizes patient information and communicates an accurate clinical presentation of the patient for accepting physicians and nurses Completes all training and is competent in the electronic applications related to job functions Adheres to regulatory requirements such as EMTALA, Ethics and Compliance policies, and quality initiatives Monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives Communicates with physicians and outlying facilities regarding all patient transfers, potential bed placements, and assists with coordinating patient movement from one facility to another Provide follow up with facilities as needed Adheres to customer service compliance Provides clinical resource information as needed to other team members in order to facilitate optimal patient outcomes Evaluates medical necessity/available services for acute level of care for incoming transfers Communicates with Case Management and Patient Access regarding incoming transfers Uses problem-solving skills to determine the course of action to generate the best patient clinical outcome, the physicians' preferences, and the transferring and receiving hospitals Adheres to established policy and procedure and standards of care Escalates issues through the established chain of command in a timely manner Utilizes effective and professional communication, acts as a liaison between physicians, patient/family, hospital staff, patient care areas, and outside agencies Communicates and uses appropriate customer relation skills Completes all mandatory department, educational and hospital requirements Adheres to current Infection Control and Safety Standards Regular and prompt attendance Ability to work schedule as defined and overtime as required Related duties as assigned Additional Information Employees must be in a regular position, working 20 hours or more per week (.50 FTE or greater) to qualify for benefits. Responsibilities Practices family centered nursing according to policies and Nurse Practice Act; serves as charge nurse when assigned; supervises, plans, evaluates, and coordinates patient care provided by other licensed and non-licensed staff; demonstrates CPR according to American Heart Association standards; abides by and enforces all compliance requirements and policies and performs these responsibilities in an ethical manner consistent with the organization's values; adheres to hospital policies including confidentiality; serves as preceptor for new employees and nursing students; utilizes the Nursing process to assess the patient, pllan individualized care, identify expected outcomes and implement and evaluate nursing interventions for patients and families; provides direct patient care according to unit specific competencies; performs therapeutic procedures and assists with diagnostic procedures required for the care of the patient; administers medications including controlled substances by all routes; administers blood and blood products; responds appropriately to emergency patient situations; starts IVs; performs activities of daily living; requires minimal supervision to safely perform patient care; when performing skilled procedures the RN can state the purpose, rationale, precautions and observations related to that procedure; utilizes proper body mechanics when moving, positioning and transferring patients without assistance or with assistance when needed for patient safety and moving equipment that is necessary to perform essential functions; provides patient and family education by planning, coordinating and delegating, as appropriate, the educational needs of each patient; completed eduaction documentation; utilizes a personal computer or terminal to access the Hospital Information System; communicates and uses appropriate customer relation skills with physicians, patients, families and healthcare team in person and via telephone; responds to overhead pages; maintains accurate and complete medical records by utilizing the correct documentation tools and procedures, adult admission history completed within unit timeframe, daily shift assessment complete with signature, completes discharge instructions/assessment, completes Pain Assessment tool, completes MAR and restraint documentation, transcribes physician orders, and supervises, evaluates and finalizies the patient care record; demonstrates the ability to provide competent patient care to the following patient populations Pediatric, Adolescent, Adults, Geriatric; cares for the patient on Intraaortic Balloon Pump (IABP), Ventricular Assist Devices (VAD); pulls arterial sheaths; performs basic EKG interpretation; performs hemodynamic monitor set-up, maintenance, monitoring and use inclusive of arterial lines, flow directed catheters, ICP lines; manages patients receiving temporary/permanent cardiac pacing; initiates and titrates medications (including emergency meds, vasoactive, antiarrhythmic, insulin infusion, sedation infusions, blood and blood components); manages patients being mechanically ventilated; uses SV02, SJ02, Camino monitors, and temperature management devices; follows unit standards for documentation i.e. alarm limits, turning, suctioning, tubing, fluid and dressing changes. Maintains accurate I & O, cumulative totals; assists with shift duty assignments or serves as a staff nurse as needed; as charge nurse identifies, organizes and directs activities to utilize resources, personnel, and materials to meet the needs of the patient and nursing staff; communicates with the Nurse Manager and Nurse Administrator, on an ongoing basis, regarding changes in unit status, patient related issues, staffing issues, Quality Management issues and other events as they evolve; acts as first line in problem resolution with other departments and any patient complaints; assists with shift duty assignments or serves a staff nurse as needed; verifies that emergency equipment is functional; regular and prompt attendance; ability to work schedule as defined and overtime as required; related duties as required. Qualifications Graduation from an accredited school of nursing and a current Alabama RN license or current Multi-State RN License in accordance with the Nurse Licensure Compact (NLC) for Alabama.
