Job Description The Hospice Liaison/Admissions Nurse is responsible for coordinating hospice admissions from hospitals, healthcare facilities, physicians, and community referral sources. Working closely with patients, families, physicians, discharge planners, and hospice leadership, this role facilitates a smooth transition to hospice services while ensuring compliance with clinical, regulatory, and reimbursement requirements. The Hospice Liaison serves as a clinical consultant, educator, and customer service resource to referral sources and assists patients and families in understanding hospice services and determining the most appropriate level of care. Principal Accountabilities Develop and coordinate the initial plan of care for patients admitted to Home Hospice or General Inpatient Hospice.
