Care at Home is a collaborative, population health-focused care delivery model serving Dual Eligible Special Needs Plan (DSNP) members-individuals with significant medical, behavioral, and social complexity, including serious mental illness (SMI) and substance use disorders (SUD). In this role, you will partner with an interdisciplinary care team to deliver high-quality, evidence-based behavioral health care. As a Behavioral Health APC (PMHNP), you will play a critical role within an interdisciplinary care team, bringing psychiatric expertise into a home-based and population health model of care. You will support both direct patient care and consultative care. If you have Georgia licensure, you will have the flexibility to work remotely* as you take on some tough challenges. Primary Responsibilities: Conduct telehealth-based psychiatric evaluations for patients with serious mental illness and substance use conditions Diagnose, treat, and prescribe evidence-based pharmacologic and non-pharmacologic interventions Provide bridge stabilization through medication management and follow-up care for high-risk patients Work with interdisciplinary care team to ensure connection to community mental health services. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
