BLS required. Ability to communicate effectively verbally and in writing. Able to manage time effectively. Bi-Lingual is a plus. Current California driver's license and proof of insurance. DEGREE OF TRAVEL: • Travel required for patient visits. • Occasional travel for educational purposes. Qualifications QUALIFICATIONS: • Must have Associates Degree, Bachelor Degree preferred. • Must have one year of acute or hospice pediatric experience • At least 12 months experience with 1-2 years preferred in acute care. • Current CA RN license. • Current CA driver’s license with proof of insurance. • Current CPR/BLS • Prefer certification as hospice nurse. • Pediatric experience preferred. • Proof of recent TB test result • Bilingual preferred. • Computer skills (word processing), use of adding machine, calculator, fax, photocopier.

Communicates with community health related persons to coordinate the care plan. POSITION QUALIFICATIONS 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses / Licensed Vocational nurse shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN / LVN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or pediatric hospice nursing is preferred.. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 5. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 6. PEDIATRIC HOSPICE RN CASE MANAGER JOB DESCRIPTION Overview The Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients.
