The LPN is responsible for assessing patient health problems and needs, implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required. ESSENTIAL DUTIES AND RESPONSIBILITIES The essential functions include, but are not limited to the following: Provide skilled nursing care under the direct supervision of the Clinical Supervisor Complete evaluation tasks, including vital signs and medication reviews Coordinate with physicians, therapists and other individuals in the client’s care plan Assess and chart observations of the client’s condition at each visit Maintain complete and accurate documentation of nursing care Work collaboratively with all members of the healthcare team Provide education to clients and families on proper home health care procedures and strategies Record patients' medical information and vital signs. Monitor, record, and report symptoms or changes in patient's conditions. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans Modify patient treatment plans as indicated by patients' responses and conditions Monitor all aspects of patient care, including diet and physical activity Assess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problems Perform other duties as assigned MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES) Associates degree required; Graduate of an accredited state-approved, Bachelor’s Degree from school of nursing preferred Current and valid Connecticut Licensed Practical Nurse license One year of LPN experience, Current CPR certification (required) Driver’s License, Car Insurance, Car Registration Strong interpersonal skills that build personal rapport and establish trust with the client Computer literate, Strong organizational skills Excellent written and communication skills Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers) Home health care experience Bilingual (Spanish) preferred PHYSICAL DEMANDS AND WORK ENVIRONMENT The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. Diagnoses are determined and prioritized from the assessment data using appropriate complex clinical reasoning. Performs or implements interventions and treatments with knowledge of health care research findings and reflecting a scientific basis and theory. Order, perform and interpret laboratory tests. The Nurse Practitioner will order and interpret any tests necessary to the medical evaluation. Examples include CBC, serum chemistry, tumor markers, chest x-ray, cat scans, nuclear scans, MRI, venous Doppler studies, EKG and Echo at appropriate oncology disease specific treatment interval as defined by NCCN guidelines. Select and performs tests and therapeutic procedures for the patient as necessary, adhering to scope of practice guidelines and program area's curriculum. Prescribe Drugs: The Nurse Practitioner will prescribe drugs as necessary as appropriate for the Hematology/Oncology office in accordance with state and federal law. Provide preventive care and diagnosis and treatment of episodic, short-term and stable chronic hematologic/oncologic problems. Identifies educational needs of patients and families and collaborates with other members of the multidisciplinary patient care team to develop programs and materials for team members, patient/family and the community. PCA shall assist residents with operating the elevator lift. PCA shall assist residents by performing housekeeping and light maintenance tasks as needed.
