Coordinate service delivery with team members, including ensuring access and securing authorization for services. Collaborate with existing providers and other collaterals and deliver services in accordance with client’s plan of care. Participate in team meetings to ensure effective communication among all disciplines involved in participant’s care. Follow the CBHC participant across the continuum of care to ensure care coordination. Partner with participant, their care team providers, and supports to ensure that care plan and safety plan are implemented as developed and make needed adjustments. Monitor participant’s progress in treatment and assist the clinical team with evaluating the need for continued services. Complete all required documentation in a timely manner meeting Agency and division established deadlines Attend assigned trainings; maintain necessary certifications and licenses (CPR/First Aid, Safety Care, MAP).
