Conducts motivational interviews that include patient screening and/or coaching on disease management ability motivation to change learning care needs and community outreach needs. Identifies potential behavioral health needs of patients using appropriate screening tools and notifies provider of findings. As part of a High-Reliability Organization reports actual and potential problems concerning patients families and associates using established reporting protocols for MedStar Health and/or work location. Participates in performance improvement initiatives to improve care quality and safety. Ensures ongoing compliance with regulatory accreditation and quality/safety standards by maintaining and auditing temperature logs ensuring medication supplies are current/unexpired checking and maintaining emergency equipment area free of clutter and other related tasks. Participates in population management activities including an awareness of the populations served and engages the patient in self-management support care/health goal setting pre-visit planning and the capturing of information that leads to health care improvement activities. Performs selected patient and laboratory testing consistent with the scope of the practice including but not limited to Point of Care Testing tuberculosis skin testing electrocardiography pulmonary function tests and vision and hearing testing (dependent upon provider and site requirements). Removes sutures/staples after the provider has examined the patient and written the appropriate order. Documents these activities in the patient's electronic medical record (EMR) or other venues based on policy. Documents clinically related activities accurately in the patient's EMR; performs required patient follow-up: to include pap smears Pb levels HIV mammograms and STD's. Collects and processes specimens completes requisitions and documents in logbook or other applicable forms/records. Provides escort for patients and assists with patient discharge activities. Provides chaperoning for patients per policy. Fulfills infection prevention and environmental responsibilities as assigned which many include: Light cleaning during clinic hours and cleaning of equipment. Under onsite provider supervision may insert intravenous catheter for physician initiated IV fluid therapy or administration of intravenous medications. May perform phlebotomy. Utilizes the EMR to propose patient medication refills for provider approval. Completes requisitions for tests (if applicable). Contacts insurance companies for Pre-authorizations for patient medications and specialty referrals including insurance authorization. Floats to other sites/locations within the system as appropriate and performs all duties at multiple sites. About this Job: General Summary of Position Professional care provider who assesses plans implements and evaluates the nursing care of patients from admission through discharge. In collaboration with the patient and family provides theory-based professional nursing care and coordinates care delivery with various members of the interdisciplinary team. Primary Duties and Responsibilities Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality standards safety standards and MedStar SPIRIT Values. Complies with governmental and accreditation regulations. Collects and analyzes significant patient's information that pertains to physical psychological socio-cultural economic spiritual and life-style behaviors and efficiently leveraging available technology. Functions autonomously providing relationship-based care that promotes continuous consistent efficient and accountable delivery of nursing care using the nursing process with a favorable focus on patient experience. Identifies current and potential complex problems of specific patient populations critically analyzes trends and changes in patient status and plans appropriate interventions. Incorporates evidence-based practice and research into patient care delivery with a focus on quality and safety. Serves as a patient advocate by independently communicating clinical concerns including ethical decisions in a timely manner to other members of the interdisciplinary team. Practices within legal boundaries of nurse practice act and possesses a theoretical understanding of the universal ethical principles. Recognizes ethical discourse in practice and may seek other resources in advocacy for support and counsel. Individualizes patients plan of care focusing on short and long-range goals and effectively uses a variety of teaching resources to meet the learning needs of patients and families. Communicates patient information clearly and accurately in verbal and written format. Maintains a safe and therapeutic environment. Identifies actual and potential patient safety issues and acts accordingly while incorporating HRO quality and safety principles. Demonstrates effective leadership behaviors. Establishes priorities and delegates tasks to peers and ancillary staff as appropriate to scope of practice and unit requirements. Effectively uses problem identification and resolution skills focusing on the clinical scenario to resolve the apparent concern. Follows the chain of command by consulting with other healthcare professionals and seeks assistance as needed. Independently communicates concerns in a timely manner and makes suggestions to members of the interdisciplinary team by seeking and using resources effectively. Evaluates the patient's response to treatment and progress toward short and long-range goals and revises nursing interventions as appropriate. Identifies patient and family needs prior to discharge. Initiates and monitors discharge planning activities in collaboration with other members of the interdisciplinary team. Independently identifies resources and strategies to enhance professional growth and development and commits to life-long learning. Supports and contributes to the unit department entity and organizational goals inclusive of the mission vision and values. Participates in meetings and on committees and represents the department and hospital in community outreach efforts. Participates in multi-disciplinary quality and service improvement teams. Minimal Qualifications Education Associate's degree in Nursing from an accredited academic nursing program required and Bachelor's degree in Nursing (BSN) from an accredited academic nursing program preferred and If prepared at the associate degree in nursing level required to enroll in a BSN program within 12 months of hie date and completion within 4 years of hire date Experience 1-2 years Prior clinical nursing experience required Licenses and Certifications RN - Registered Nurse - State Licensure and/or Compact State Licensure Current license as a Registered Nurse in the state or jurisdiction of the facility seeking employment Upon Hire required and CPR - Cardiac Pulmonary Resuscitation (includes BLS) for healthcare providers from either the American heart Association (AHA) or American Red Cross Upon Hire required and Additional unit/specialty certifications may vary be department or business unit Knowledge Skills and Abilities Basic math skills Verbal and written communication skills Basic computer skills required This position has a hiring range of : USD $35.68 - USD $64.71 /Hr.

Interventional Cardiology Nurse Practitioner

We are looking for a Nurse Practitioner or Physician Assistant to join our Interventional Cardiology team at MedStar Union Memorial Hospital Center with coverage at MedStar Franklin Square Medical Center. This will be a full time, 36 hr/week position mainly focused at MedStar Union Memorial Hospital Center with possible availability to cover at Medstar Franklin Square Hospital. In this role, the PA/NP would be primarily responsible for pre procedure prep and post procedure recovery of Interventional Cardiology patients. They will be working in a well-staffed facility with a state-of-the-art lab. This provider will have immediate access to very experienced APP peers and physicians to learn and grow in their career. The scheduling for this role will mainly be (3), 12 hr. shifts a week, with occasional shorter shifts depending on rotating weekend shift. There may be opportunity for later coverage hours, but no nights. Minimum Qualifications: Education/Training Graduate from an accredited Physician Assistant or Nurse Practitioner Program.
