The primary function of the Adult Day Care (ADC) Aide is to provide your assigned registrants with routine daily care in accordance with our established care procedures as may be directed by your supervisor. Completing and performing your assigned tasks, must report any abnormal findings to the ADC Coordinator or designee. Perform all assigned tasks in accordance with the established policies and procedures and as instructed by your supervisor. Notify the facility when you will be late or absent from work as well as the ADC Coordinator. Report all complaints and grievances made by the registrant or registrant's family members. Cooperate with interdepartmental personnel, as well as other facility personnel to assure that services can be adequately maintained to meet the needs of the registrants. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as common environment throughout your assigned shift. Participate in and receive a report upon reporting for duty. Perform those nursing functions as assigned by the ADC Coordinator. Report to duty on time and in proper attire. Practice proper infection control techniques. Complete assignments by end of shift. Attend to registrant's needs promptly Assist other Adult Day Care personnel as needed. Maintain a safe and clean environment for all registrants assigned to your care. Demonstrate the basic techniques of rehabilitation. (Ex; Range of motion, activities of daily living, ambulation and transfer techniques). Attend in-service programs as directed by the ADC Coordinator. Maintain records as directed. Check registrants routinely to ensure that their care needs are being met. Report any changes in room temperature, ventilation, lighting, etc. to the appropriate personnel. Assist registrants in preparing for meals and assist in serving meals to the registrants. Assist in serving between meal snacks. Pick up and clean environment after activities and meal time. Maintain a safe, attractive and clean work area. Operate all equipment in a safe manner. Maintain the confidentiality of all registrant care information. Ensure that you treat all registrants fairly, and with kindness, dignity and respect. Ensure that all care is provided in privacy as appropriate. Report all grievances and complaints made by the registrant to the ADC Coordinator. Must possess a valid NYS driver license without violations. Drive and/or monitor registrants on the facility van when transporting registrants to and from the Adult Day Health Care Program. Nurse Aide must attend to registrant's needs and safety. Nurse Aide must learn and use proper van hook-ups. Nurse Aide must supervise registrants on all outings. Nurse Aide must follow activity schedule and execute all plans on monthly calendar. Nurse Aide must initiate and continue activities. Nurse Aide will put all equipment and materials away at end of shift. If Adult Day Health Care closes for a snow day or another type of emergency day, the Adult Day Health Care Nurse Aides must report to the facility Nursing Supervisor for an assignment on the Nursing units. If Adult Day Health Care census is deemed low, the ADC Coordinator will assign Nurse Aide to the Nursing Home, Nurse Aide must report to the facility Nursing Supervisor for an assignment on the Nursing units. Attend all in-services and follow all department and facility policies including the use of personal protective equipment. Maintain CONFIDENTIALITY of all pertinent resident care and employee information and follow HIPAA and HIV regulations. Pay range: $29.13 to $38.00 (depending on experience, shift, benefit status) Position Overview: In this role, you will be responsible for providing high-quality patient care by executing nursing processes, performing treatments, and supporting patients' physical and emotional needs. You will also work closely with our CNA team to ensure timely, accurate care, while managing medication administration and maintaining thorough documentation. LPN Responsibilities: Patient Care & Service: Promote and restore patients' health by completing the nursing process; perform various treatment procedures; provide physical, educational, and emotional support to patients, friends, and families; supervise assigned team members Documentation: Ensure concise, pertinent, and complete resident care documentation using computerized medical record process Supports CNA team: Direct the CNA in the timely completion of quality individualized personal care and ensure CNA documentation is accurate and complete Medication Administration & Reporting: Administer medication and give/receive relevant reports, both written and verbal from/to appropriate staff as per departmental protocols LPN Requirements Include: Current and valid NY LPN License Experience in a Nursing Home or Hospital is a plus Valid CPR Certification What you can expect from us: Stable opportunity with a wide array of experiences to further develop your career.
