The Registered Nurse provides care consistent with the standards of professional nursing. Provides leadership and supervision to CNA/SNA and other healthcare workers as assigned. Requirements: Currently licensed as a Registered Nurse in the state of Illinois. Previous RN experience in specialty area required. CPR and any additional certifications for specialty area. FULL TIME (0.9)— minimum of three (3) twelve (12) hour scheduled shifts Friday, Saturday and Sunday OR Saturday, Sunday, and Monday every week. Weekend Warriors are allowed one weekend off per quarter. (Quarters based on calendar year January 1 through December 31). Weekend off requires manager review and approval. Two weekend shift trades are allowed per quarter. The employee may not carry over unused trades or their weekend off into the next Weekend warrior program. Trades require manager review and approval. F/T employees will remain on their holiday rotation. If a holiday falls on a weekday, the employee will work their holiday and be taken off a normal scheduled weekend shift. P/T employees will be required to work the holiday if it falls on the weekend. When PTO is used it will be paid at the base rate. There will also be a contract you will have to sign for the agreement. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Chicago suburbs (Joliet-Palatine) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow. Administer medications within the scope of practice of the R.N. Licensure. Maintain a current report of narcotics received and used. 6. Review daily the documentation of the dispensing of the controlled substances and narcotics. Ensure that drugs covered by controlled substances laws are verified by inventory. 7. Place pharmacy orders, for and administer all newly prescribed medications and document. 8. Carry out direct contemporaneous charting in your shift. Chart detailed monthly evaluations of each Guest that reflect his/her condition and progress. 9. Responsible for all nursing care of assigned Guests while on duty. Must notify appropriate persons if there is any significant change in a Guest’s condition or any transfer to a hospital. 10. Ensure that Guest care plans are being followed and asses each Guest’s status in accord with their care plan. 11. Round with physicians when needed. 12. Administer or supervise all treatments prescribed by physicians including but not limited to pressure ulcer care, Foley catheter care, hot and cold compounds and intravenous therapy. 13. Supervise serving of prescribed diets and fluids intake and immediately report any issues to the physician and DON/ADON. 14. Must be knowledgeable of individual care plans and support the care planning process by reporting specific information and observations of the Guest’s needs, preferences and report any behavioral changes. 15. Provide wound care when needed. 16. Completes medical records documenting care provided and other information in accordance with nursing policies while maintaining strict confidentiality. 17. Assure that established infection control and prevention practices and standard precautions are maintained at all times. 18. Follow established safety precautions when preforming tasks and using equipment and supplies. 19. Reports all hazardous conditions, damaged equipment, accidents/incidents and supply issues to appropriate persons. 20.
