Staff Nurse - Ambulatory Surgery Center - 36hr

Under general supervision, assumes 24-hour accountability to plan and provide direct professional nursing care for specified patients throughout the entire episode of service delivery as defined by population and unit. Provides nursing services to patients and families in accordance with the standards of practice of a professional nurse, CCMC policies & procedures, Joint Commission, and State & federal regulations. Reports to Nurse Manager. CLINICAL PRACTICE - On a daily basis, demonstrates clinical expertise and uses the nursing process to provide safe, appropriate patient care to a defined patient population according to CCMC and unit policies and standards. POPULATION SPECIFIC - Demonstrates knowledge of the differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families. UNIT LEADERSHIP - Delegates duties and tasks associated with patient care to clinical staff according to hospital policy and their scope of practice. As assigned, contributes to the leadership of the unit by functioning as the Resource Nurse and/or by participating in the professional development of others through preceptor/mentor activities. Supports the unit/department’s work productivity and resource management activities. DOCUMENTATION/COMMUNICATION - Completes all required documentation in a timely manner, according to CCMC policies and standards; necessary information is effectively communicated to all members of the health care team. PATIENT/FAMILY PARTNERSHIP & EDUCATION - Partners with child and family in evaluating outcomes. Monitors, trends, and evaluates patient/family responses to treatment. Provides appropriate education to patients and families regarding all aspects of care according to CCMC policies and standards; supports the organization’s efforts to patient/family education. CCMC STANDARDS - Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the CCMC Standards. Education and/or Experience Required: A minimum of ASN/ADN required upon hire, BSN preferred. If ASN/ADN, must acquire BSN within 3 years of hire. Minimum of 2 years recent perioperative experience. License and/or Certifications Required: Current State of Connecticut Registered Nurse Licensure required or nurse licensure compact (NLC) and maintained thereafter. Current Basic Life Support (BLS) / CPR in adult, child & infant is required upon hire.+ Advanced Cardiac Life Support (ACLS) is required within six months of hire. Specialty licensure/certification may be required, as specified by department. Licenses and/or Certifications Preferred: Certification in OR nursing is preferred. Pediatric Advanced Life Support (PALS) required within 6 months of hire or before staff has completed dept. orientation if this is longer than 6 months. Primary Responsibilities: Clinical Competence Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations Provides required supervisory visits Documentation and Care Delivery Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care, and in coordination with other health care team members Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely Documents patient visits per policy and payer requirements, and syncs timely per LHC policy Quality Makes initial and/or comprehensive nursing evaluation visits, ensures patients meet home health eligibility and medical necessity guidelines, determines primary focus of care, develops the plan of care within State guidelines with the physician, and submits accurate documentation Communicates relevant information timely and effectively with appropriate agency staff, including patient care issues, visit assignments, schedule changes, orders, OASIS data sets, coding requests, and coordination with other clinicians Communicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence Teamwork Takes direction from Clinical Director and Executive Director professionally and completes assigned tasks timely, including required learning Assists in the orientation of new agency personnel and serves as a preceptor to other staff and students Actively participates in survey/survey readiness activities and performance improvement plans, works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes Participates in on-call and weekend rotation as needed to meet patient needs Adheres to and participates in the agency's utilization management model You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of 'direct' and 'indirect' patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable.
