Proactively serves as a patient advocate by utilizing courteous and professional etiquette at all times reflecting positive tone and speaking distinctly with poise, tact and assurance. Assures accurate and complete messages to appropriate personnel . Assists in training, orienting, and supporting new colleagues. Work includes cross coverage in other clinic areas as team needs. Document patient plan(s) of care, tests and examination results in the medical record as directed by the provider. Escalate non-routine issues, questions and/or concerns to the practice manager or healthcare provider(s). Ensure safety checklists/quality controls are completed as required . Other duties as needed and assigned by the manager. MINIMUM QUALIFICATIONS Must be comfortable operating in a collaborative, shared leadership environment. Successful completion of a medical assistant program accredited by the Commission on Accreditation of Allied Health Programs (CAAHP) or Accrediting Bureau of Health Education Schools (ABHES). In collaboration with the physician, Population Health Case Manager, and patient, develops appropriate assessment-based plan of care including: attainable patient goals and services needed to improve and/or stabilize patient’s medical condition, functional abilities and promote independence. Maintains a current working knowledge of applicable Federal, State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and follows Code of Conduct. Reports changes in patient condition in a timely manner to the physician and/or Population Health Case Manager. Obtains orders for changes in the plan of care to respond to the patient's condition. Follows orders for treatment and provides services as indicated on the plan of care. Employment Type: Full time Shift: Description: RN Case Manager (Onsite) Shift: Monday-Friday (8 hours) No Weekends General Summary: Responsible for the review of inpatient and outpatient admission records for appropriate admission status at Mercy Medical Center and Mercy West Lakes. Works in collaboration with the attending physician and the Case Management staff utilizing admission criteria guidelines-and second level physician review process when appropriate. Interacts with insurance providers to obtain authorization and continued stay approval for admission. Collaborates with the Verification department, Revenue cycle and Medical Eligibility to facilitate the establishment of the correct payer source for patient stay and the documentation of the interactions in the STAR admitting system Key Responsibilities Perform admission, concurrent, and post-discharge utilization reviews in accordance with the Utilization Management Plan and regulatory requirements. Apply Milliman Care Guidelines and payer-specific criteria to determine appropriate patient status. Collaborate with attending physicians to clarify admission status and initiate second-level physician reviews as needed. Communicate status changes promptly to Case Management, Admitting, and other relevant departments. Provide patient/family education and issue Notices of Status Change when required. Accurately document utilization review activities, status determinations, authorizations, denials, and communications in the medical record and STAR system.
