MAJOR FUNCTIONS: Under general direction, the Nurse Practitioner is responsible under direction of a physician for providing general medical care, preventative care and treatment to patients in one of the agency’s clinics. This includes diagnosis, treatment, consultation and follow-up care. This individual attends to the patient’s need with the utmost patient care, compassion and understanding. This work is carried out in support of the mission. DUTIES AND RESPONSIBILITIES: Performs routine physical examinations and preventive health measures within prescribed guidelines and instructions of a physician based on assigned program. Orders, interprets and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs. Records accurate medical histories, physical findings and formulates plan and prognosis based on patient’s condition. Documentation includes medical evaluations, diagnosis, procedures, treatment, prognosis, referrals, education and consultation. Discusses case with physician and other health professionals and therapeutic team to prepare comprehensive patient care plan. Submits health care plan and goals of individual patients for periodic review and evaluation by physician. Prescribes medications and monitor effectiveness, or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures. Refers patients to physician or to specialized health resources for treatment. Makes patients aware of importance of a healthy lifestyle. Educates patients and their families on their preventive care, medical issues and use of prescribed medications and treatments. Analyzes new data gained from conferences, training workshops, professional literature and on the job training, where they will assimilate it into their clinical work. Keeps abreast of changes in field. Performs other duties as requested. REQUIREMENTS: Education, Experience and Skills Required: Master’s degree from an accredited advanced Nurse Practitioner program At least Two (2) years’ experience working as a Nurse Practitioner preferably in addiction medicine or substance use and/or mental health treatment programs Knowledgeable about federal and state regulations governing medical/clinical treatment in substance use treatment programs Ability to demonstrate sensitivity to patients with a variety of psycho social issues License/Certification Required: NYS RN and NP (Adult and Family specialty areas) licensures. Job Description Registered Nurse (RN) – Surgical Oncology Physician Practice Position Summary The Registered Nurse (RN) is a vital member of the multidisciplinary surgical oncology team, providing comprehensive nursing care to patients throughout the continuum of surgical cancer treatment. The RN collaborates closely with surgeons, advanced practice providers, medical assistants, and ancillary staff to coordinate patient care, provide education, facilitate treatment planning, and ensure exceptional clinical outcomes. This role requires strong clinical judgment, excellent communication skills, compassion, and the ability to manage complex patient needs in a fast-paced outpatient setting. Essential Duties And Responsibilities Clinical Care Perform comprehensive nursing assessments and triage patients in person and by telephone. Evaluate patient symptoms and determine appropriate interventions or escalation of care. Monitor patients before and after office-based procedures. Review laboratory, pathology, and imaging results, communicating findings and provider recommendations to patients. Coordinate preoperative and postoperative care for surgical oncology patients. Complete disability forms, FMLA paperwork, and other clinical documentation as appropriate. Telephone Triage Assess patient concerns using evidence-based triage guidelines. Provide clinical advice within scope of practice. Escalate urgent or emergent concerns to the provider. Document all patient communications in the EMR.
