Plan of care/Documentation/Patient Family Centered Care/Patient Experience Develops, discusses, and communicates a prioritized problem list/plan of care for each patient. a. Develop, evaluate and update individualized plan of care for patient and documents outcomes. b. Initiating admission assessment within 8 hours of admissions, identifies and documents patient/family/significant other teaching needs upon admission and throughout hospitalization. c. Continues assessment/reassessment and identifies care needs within established nursing practice. d. We are hiring a full-time MDS Coordinator to join our dynamic care team at Southeast Rehabilitation & Skilled Care Center in North Easton! Can be LPN or RN. Serves as a single point of contact for referring physicians, clients, caregivers, and human service organizations to provide resources and assistance with behavioral health care services offered within South Shore Health System and its partnering healthcare community. The BHN will provide brief interventions in Acute Care, primary care and other community settings, educate clients on the importance of MH services, and navigate clients to a long-term provider. Connect with referred clients in the acute care setting, during PCP visits or in other community settings skillfully and effectively. 3. Assess presenting problems efficiently, and developing an initial treatment plan with the client or family seeking treatment. 4. Assist clients in understanding their diagnosis, treatment options, and the resources available, including educating eligible clients about appropriate community services, clinical research studies, and technologies. 5. Provide brief behavioral health interventions in the form of Individual, Family, or Group treatment. Possesses knowledge of substance abuse assessment, recovery principles, behavioral activation, and chronic disease self-management. 6. Complete intake of high-risk clients, working with the client, family and other members of the healthcare team as needed, to assess client’s behavioral health needs, physical health, support system, financial resources, and available community and government resources. We are hiring a part-time Staff Development Coordinator to join our team at Southeast Rehabilitation & Skilled Care Center in North Easton! This is an in-person position at our facility. Job Description Balance Life & Work with a New Career Opportunity (LONG TERM CARE) (SHORT TERM REHAB (SKILLED NURSING) Now Hiring – Weekend Registered Nurse – Randolph, MA Min $45.00/hr- Max $51.00/hr Compensation will be based on, but not limited to, experience, qualifications, credentials, employment status, and work schedule CareOne at Randolph Responsibilities The Weekend Registered Nurse is responsible for the Shift coordination and oversight of the nursing process for residents assigned to his or her care. Responsibilities include both the direct and indirect provision of care according to each resident's nursing and medical plan of care and in accordance with state and federal regulations and accepted nursing standards of practice. Job duties include, but are not limited to: Provide direct nursing care to assigned residents in accordance with the nursing process. May be assigned to function as Medication/Treatment Nurse, unit Charge Nurse, or Supervisor. Direct day to day functions of Licensed Practical Nurses and Certified Nursing Assistants, assuring compliance with state and federal regulations and facility policies and processes. Provide assessment of and responds to resident conditions utilizing the nursing process. Work cooperatively with other members of the facility interdisciplinary team. Maintain effective communication with other members of the interdisciplinary team utilizing facility processes in order to assure the highest standard of care is maintained for each resident assigned to his or her care. Complete and ensure LPN and C.N.A. documentation of care and services rendered according to facility policies and procedures. Complete reports such as the 24-hour change in condition report, Incident and Accident reports, etc. according to the facility policy and procedure and as assigned by the Director of Nursing Services. Provide immediate response to situations of resident risk such as allegations of abuse, neglect or misappropriation of property. Report incidents timely and to appropriate persons per facility policy. Ensure timely and accurate medication and treatment administration. Communicate to physician and documents changes in resident condition utilizing both objective and subjective nursing assessment. Document and implement changes in nursing and medical plans of care. According to facility policy and procedure, assure that all members of the interdisciplinary team are aware of and understand changes in resident nursing and/or medical plan of care. Position Requirements Associate's degree from credentialed school of nursing.
