Oversee medical and administrative equipment by verifying the availability of emergency equipment, completing preventive maintenance, troubleshooting malfunctions, and coordinating repairs. Maintain a cooperative relationship among healthcare teams by communicating information, responding to requests, building rapport, and participating in team problem-solving. Requirements: Current NYS RN Licensure (BSN preferred but not required). Minimum two years of experience in Long-Term Care or medical/surgical settings. medication administration records (MAR) for completeness of information, accuracy in the transcription of physician orders · Inspect nursing treatment areas daily, such as clean utility room, to ensure that they are maintained in a clean and safe manner · Meet with family members and resident as needed. · Consult with the resident, family, and physician in planning the resident's care, treatment and rehabilitation · Assist in arranging transportation for transfers to the hospital · Participate in the implementation of facility quality improvement program for the nursing service department · Participate in facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing · Attend and actively participate in various Director of Staff Development in providing off shift in-services as necessary · Assist in the Quality Assurance and Performance Improvement (QAPI) process QUALIFICATIONS: EDUCATION / EXPERIENCE: · Bachelor of Science in Nursing (preferred) · Current registration and license to practice as a Registered Nurse in the state employed in · Peripheral IV certification · Two years of clinical nursing experience · One-year experience in a management position preferred · One-year experience in geriatrics and subacute population · CPR/BLS certified · The ability to read, write, speak, and understand As a Post Acute and Long Term Care respiratory therapist, your role would involve providing specialized care and treatment to patients with cardio-pulmonary, respiratory conditions neurological disorders and physical conditions that manifest into respiratory disorders over an extended period. Your primary responsibility would be to assess, provide diagnostic testing, develop treatment plans, and provide treatment to our Skilled Nursing Facility population. Job Description ESSENTIAL JOB FUNCTIONS: 1. Provides patient treatments which meet patient needs and uses current treatment knowledge in accordance with facilities clinical procedures. This includes but is not limited to: · Performing accurate, comprehensive patient evaluations. · Developing an effective plan of treatment and obtaining appropriate approvals from a referring physician. · Communicating with supervisor and interdisciplinary team members regarding patient progress, problems, and treatment plans. · Instructing patient’s families and/or nursing staff in specified follow through programs. · Interviewing patients and families regarding previous level of functioning, lifestyle and current and future expectations for recovery. · Demonstrating knowledge of appropriate resources for problem-solving and clinical best practice. · Evaluating patients within the required timeframe as per facility policies. · Provides patient care as appropriate and necessary, following the position description for the applicable clinical discipline in which licensure or certification is held. 2. Includes specific, objective measurable goals. 3 . Consistently demonstrates facility interdisciplinary approach to patient care. This includes but is not limited to: Integrating findings of other members of the interdisciplinary team in treatment assessment and recommendations. Integrating input from patient and family in treatment assessment and recommendations. Preparing for and providing meaningful contributions to the rehab team conferences, patient care conferences, utilization review meetings, family conferences, and caregiver training sessions. Collaborating with and informing facility staff in areas of patient progress. Educating members of the team in areas of expertise. Working with a facility to teach documentation practices which assist the respiratory therapy team. Communicating effectively with discharge planners. 4. Daily Pay Some of your Licensed Practical Nurse (LPN) Job Responsibilities include: Admissions, Discharge and Transfers of residents Medication requisitions with pharmacy and physicians Medication administration Following infection control, emergencies, and other center policies Collaborating with resident’s interdisciplinary team to provide the best outcomes Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment. Delivers nursing care to patients/residents requiring long-term or rehabilitative care. Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident. According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response. In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations. Maintains knowledge of necessary documentation requirements. Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc. Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information. Licensed Practical Nurse (LPN) QUALIFICATIONS: LPN (Licensed Practical Nurse) licensed by the State CPR Certification required Two Years prior nursing experience preferred
