Responsible for conducting and coordinating the development and completion of resident assessments, in accordance with the requirements of State, Federal and Company guidelines. Responsible for overseeing the generation of Minimum Data Set (MDS) for each Medicare patient and electronic transmission of required data within time frame mandated by the State. Coordinates the development and completion of the resident assessment (MDS) in accordance with current regulations and guidelines, including; the implementation of CAAs and Triggers; conducting or coordinating the interview(s) of each resident for the resident’s assessment; evaluating each resident’s condition and pertinent medical data; developing and implementing procedures with the Director, Clinical Services for arrival of newly admitted residents; ensuring that all assessments are completed and transmitted in a timely manner; assisting community directors and supervisors in scheduling the resident assessment/care plan meetings; and contacting and assisting in scheduling participation by outside members of the care plan team, including the resident’s representative and/or other interested family members. Degree from an accredited Nursing Program. Current State Registered Nurse License. Current CPR Qualifications Required Skills: Successful completion of accredited Registered Nursing program required. Bachelor of Science in Nursing degree preferred. Current state license as a Registered Nurse required. Current CPR Certification required. One (1) year recent experience preferred. Acute care setting preferred. ACLS certification preferred in LTACH and Texas hospitals. Additional Qualifications/Skills: Knowledge of and adherence to current nursing theory and practice, infection control, and wound care standards of practice. Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS. Knowledge of clinical operations and procedures, and utilizes proper policies, procedures, and guidelines in caring for patients. Demonstrates critical thinking skills. Demonstrates general computer skills including: data entry, word processing, email, and record management. Effective organizational and time management skills. Ability to maintain quality, safety, and/or infection control standards. Responsibilities The Registered Nurse provides professional nursing care to patients within assigned unit. The role is also responsible for patient assessments, planning, supervising, interventions, and evaluations designed to prevent complications of physical disability, restore optimal functioning, and help the patient adapt to an altered life style. It’s not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case.
