Job Responsibilities: Manage all aspects of patient care from initial admission assessment to weekly visits, to discharge when goals are met. Coordinate patient care with their physician as well as other disciplines involved in care (PT,OT,ST,HHA) Upon admission, develop plan of care tailored to patient’s individual needs, including establishing goals and discharge planning. Revise the plan of care as necessary, depending on patient status and any changes. Communicate with physician regularly on patient condition and to request any orders necessary. Provide direct nursing care and education based on the care plan. Completes all assessments and nursing notes timely and thoroughly. Collaborate with other home health disciplines for the best patient outcomes, which includes attending case conference meetings regarding patient overall care. Supervision of HHA and LPN visits, when applicable. Communicate effectively with other team members, including administrative supervisors regarding scheduling, patient updates, or any other daily updates. Participate in on-call rotation when applicable. Qualifications: Graduate of an accredited school of nursing with Associate/Bachelor’s degree. Current unencumbered LPN license and CPR certification 1 year of home health experience preferred, but willing to train the right candidate. 1-2 years of acute care LPN experience required. How you’ll make an impact in this role You will be working in a high-acuity, 46 -bed unit where the patient-to-nurse ratio is 1:3-4 Patients come from the ER or are transferred from the Trauma Neuro ICU and MICU. Job Summary The Perianesthesia Nurse is a registered nurse responsible for the care of patients being prepared to undergo, or recovering from, anesthesia. The Perianesthesia Nurse is tasked with the medical management of each patient and has the skill and knowledge to address critical medical emergencies pre- and post-surgery. Job Responsibilities Prepare patient for surgery or procedure. Ensure the patient chart is initiated, complete the patient assessment, confirm the procedure and/or surgical site, initiate patient IV, perform ordered labs, dispense ordered preop medications and fully document all information and care provided. Provide instructions to patient and support persons as to what to expect throughout the surgery process. Coordinate patient care needs with other disciplines to ensure continuity of care. Monitor patient recovery from anesthesia and medical procedures. Regularly monitor and record patient vital signs, including heart rate, blood pressure, temperature, and respiration rate.
