Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (NP) for True Vine Family Medicine at 201 N. Plaza Blvd., Chillicothe, OH 45601 The APP, in collaboration with the Physician, coordinates the health care of primary care patients at this location. This position monitors the patient’s status and collects data to evaluate and improve care by providing a comprehensive assessment, implementing the appropriate treatment, and evaluating the patient’s progress. Key Responsibilities: Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients of all ages.
