Job Title/Position : Hospice On Call RN Reports To : Clinical Director JOB DESCRIPTION SUMMARY : The On-Call Hospice RN is responsible for responding to all of the Hospice patient census and their families, as well as handling any referrals or community inquiries outside of office hours (nights, weekends and holidays). Job Description Nemours is seeking a Registered Nurse for our general pediatrics stepdown unit to join our team in Wilmington, Delaware . This position is FULL-TIME, 72 hours biweekly, 12-hr rotating shifts, with every third weekend and holiday rotation . 3 East is a surgical intermediate unit with a high population of patients requiring post-operative care for urological issues, traumatic injuries, and general surgery. 3 East also cares for patients with acute and end stage renal disease and patients requiring an intermediate level of nursing care due to their tracheostomy and/or ventilator needs. Each patient room is equipped with oxygen, compressed air, and suction along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the healthcare team to access a patient’s electronic medical record. The patient care complexity directly relates to the wide range of patient care skills required for this diverse patient population. The goal of this unit is to provide high-quality, cost-effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set. Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPap respirators, Oxygen delivery methods, PCA & epidural pumps, wound vacutainers, ventilators and specialty beds. Nursing competencies include: chest tube management, pain management, blood product administration, urinary catheterization, tracheostomy care, ventilator management, and central line care and management. Job Description Nemours is seeking a Nursing Assistant for our 3 West floor! Position is full-time, 60 hrs every 2 wks. Rotating shifts, every 3rd holiday, two weekends out of six. This position is responsible for: Assisting with general activities of the patient care unit and assist the health care team with performance of patient care activities, under the direction and guidance of a Registered Nurse. 3 West is a 24-bed medical surgical unit for patients 0-21 years of age. This unit is designated for orthopedic patients, patients in need of comprehensive rehabilitation and acute rehabilitation of both traumatic brain injury and spinal cord injury, ventilator-dependent patients, and patients with tracheotomies. Additional patient populations include neurosurgical and nonsurgical neurology, and infants transitioning from the Neonatal Intensive Care Unit. Neurology and neurosurgical patients are also cared for on 3 West. This unit was designed to provide single patient care rooms to allow for privacy and family presence during the hospital stay. Responsibilities Ensures an adequate stock of all necessary equipment and supplies in designated areas at established par levels. Ensures a clean and safe patient care unit. Assists patients with activities of daily life (bath, feed, play, etc.) and provides other patient care as directed. Assists other staff in examination or treatment procedures including positioning, holding and comforting patients. Assures all supplies are accurately charged. Contacts pharmacy, dietary, and other departments as designated by the Registered Nurse. Collects patient specimens (blood, urine, stool, other per policy), paying close attention to patient safety, comfort and accuracy of information. Uses Hospital Information Systems as required for patient care, (e.g. documenting vital signs and patient care activities.) Accurately relays information from physicians, nurses, laboratory personnel and others. Transports patients and delivers reports, supplies, etc., to other patient units or Nemours/AIDHC locations as needed. Transports medications from Pharmacy or pneumatic tube system to an appropriate licensed personnel in the patient care area for immediate patient use or storage in the Medication Room on the patient care area. Promotes patient safety and participates in activities that adhere to patient safety standards and procedures. Protects patient privacy and maintains confidentiality. Courteously and professionally covers patient care unit front desk as needed. This includes promptly answering telephones, greeting patients, sending reports, distributing, sorting, or processing mail, entering orders, managing medical record, etc. Responsible for participation in departmental and hospital programs for Quality Assessment and Improvement, identifying opportunities to improve services, making recommendations and implementing actions as appropriate and consistent with the goals of the Nemours Foundation. Participates in unit, departmental and hospital education programs. Attends unit/departmental meetings regularly. Demonstrates proficiency in the care of patients of all ages; neonates, infants, toddlers, school aged, adolescents and adults. Job Title/Position : Hospice On Call RN Reports To : Clinical Director JOB DESCRIPTION SUMMARY : The On-Call Hospice RN is responsible for responding to all of the Hospice patient census and their families, as well as handling any referrals or community inquiries outside of office hours (nights, weekends and holidays). The on-call nurse will provide urgent clinical and psychosocial interventions for patients and their families, skilled pain and symptom management and collaboration with providers within and outside of the hospice team to maximize positive patient outcomes. The work of the On-Call Hospice RN provides considerable consideration for independent assessment and judgement in crisis situations, utilizing and consulting with the Hospice Medical Director and administrator on call as indicated. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES : Consistent availability by telephone and email during assigned shifts. Conducts urgent physical and psychosocial assessments of the patient and family during after hours and determines appropriate interventions. Provides and maintains a safe environment for the patient. Provides professional nursing care by utilizing all elements of the nursing process. Provides skilled nursing care focusing on pain control and symptom management. Provides interim guidance to the patient and family regarding diagnosis, medications, treatments and progress. Documents patient care thoroughly and timely. Completes, maintains and submits accurate and relevant clinical documentation regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
