Hiring Immediately for Admissions Registered Nurse 7pm - 7am **The available shifts are as follows: Three (3) 12-hour shifts weekly including every 3rd weekend). Additional shifts can be picked up if desired** The Admissions Registered Nurse (RN) is responsible for completing comprehensive clinical reviews and assessments to evaluate prospective patients for inpatient behavioral health admission. This individual must be extremely detail oriented. Our Admissions RN ensures timely, accurate and compassionate admission processes, including evaluating medical and psychiatric stability, obtaining health histories and coordinating with interdisciplinary treatment team. The Admissions RN may also complete insurance and financial reviews for prospective patients when needed to promote timely throughput. Duties May Include: Review referrals to ensure patient is medically stable for transfer Performs an in-depth, systematic, preadmission patient assessment and communicates pertinent information to appropriate healthcare providers Identifies any risks associated with the patient Confirms all legal paperwork is obtained for involuntary admissions Communicates with the sending facility to obtain any information needed in order to admit a patient safely Cross trains to be able to complete a referral in its entirety: financially, medically and legally Organizes and prioritizes multiple complex tasks in a fast-paced environment Works with Unit Managers/Supervisors to make bed assignments Builds positive relationships with nursing units, nursing managers/supervisors and nursing leadership to ensure the appropriate placement of patients based on priority, acuity and care needed Documents patient information into Expanse Provides hand-off communication to the receiving nursing unit Gives and receives shift reports and answers telephone inquiries Qualifications: Active New Jersey Registered Nurse (RN) License 3+ years inpatient behavioral health nursing experience required Admissions experience preferred (Med-Surg, Emergency Dept, Behavioral Health) BLS/CPR for health care provider required

North and South 8 have telemetry capabilities for our patients. Kelemen 8 has frequent admissions and discharges. Kelemen 8 can accept a total of 77 patients. The North side houses 37 patients, and the South side houses 36 patients. There are two negative pressure rooms with an anti-room. There is a pharmacist present on the unit to assist the RNs with medication questions/concerns. Multidisciplinary rounds occur every Monday through Friday. Rounds involve all aspects of the healthcare team, including the patient and family. Job Description Nemours is seeking a Nursing Assistant for our 3 West floor! Position is full-time, 60 hrs every 2 wks. Rotating shifts, every 3rd holiday, two weekends out of six. This position is responsible for: Assisting with general activities of the patient care unit and assist the health care team with performance of patient care activities, under the direction and guidance of a Registered Nurse. 3 West is a 24-bed medical surgical unit for patients 0-21 years of age. This unit is designated for orthopedic patients, patients in need of comprehensive rehabilitation and acute rehabilitation of both traumatic brain injury and spinal cord injury, ventilator-dependent patients, and patients with tracheotomies. Additional patient populations include neurosurgical and nonsurgical neurology, and infants transitioning from the Neonatal Intensive Care Unit. Neurology and neurosurgical patients are also cared for on 3 West. This unit was designed to provide single patient care rooms to allow for privacy and family presence during the hospital stay. Responsibilities Ensures an adequate stock of all necessary equipment and supplies in designated areas at established par levels. Ensures a clean and safe patient care unit. Assists patients with activities of daily life (bath, feed, play, etc.) and provides other patient care as directed. Assists other staff in examination or treatment procedures including positioning, holding and comforting patients. Assures all supplies are accurately charged. Contacts pharmacy, dietary, and other departments as designated by the Registered Nurse. Collects patient specimens (blood, urine, stool, other per policy), paying close attention to patient safety, comfort and accuracy of information. Uses Hospital Information Systems as required for patient care, (e.g. documenting vital signs and patient care activities.) Accurately relays information from physicians, nurses, laboratory personnel and others. I. Qualifications Skilled Nurse from an Accredited program currently licensed and in good standing with the Pennsylvania State Board of Nursing. One to 2 years’ experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required. II. Responsibilities It is the responsibility of the Visit Nurse to coordinate and provide the highest level of hospice care to patients and families. The Visit Nurse works in conjunction with the Nurse Case Manager, Director of Nursing (DON) and the Interdisciplinary Team to: · provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient’s care. · assess and coordinate a pain/symptom management program formulated with the participation of family and patient, attending physician and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life. · assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues. · provide support and education for families based on the hospice philosophy of care. Care includes physical, psychological, spiritual, and financial comfort. · participate in Interdisciplinary Team meetings and up-dates the Plan of Care (POC) for assigned patients based on identified problems and needs. · ensure all services provided are in accordance with the patient’s POC. · ensure the POC involves the active participation of the patient, family, PCP, and the Interdisciplinary Team · provide Nurse Case Manager with appropriate information to coordinate all services related to hospice care for assigned patients. Coordination of services include communicating and making referrals for home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services, as well as ordering medications and Durable Medical Equipment · educate patient and family on the disease process and expectation related to the disease progression. · assist with CQI activities. · work as a team member at all times and uphold the Hospice Code of Ethics · provide care to manage patient symptoms. · utilize the medication formulary whenever possible for the management of patient symptoms. · reports to DON as patient condition changes, changes in the patient condition that require change in level of care. III. Orientation and TRAining Tender Care Hospice will provide orientation and training, as necessary, to assist the Case Manager Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Case Manager Nurse will be on call according to scheduling. During the hours when on call, the Visit Nurse is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V. Accountability Reports directly to the Nurse Case Manager and Director of Nursing. The Nurse Case Manager will work under the direction of the Director of Nursing.
