Will be responsible for oversight of day to day medical coordination, assist with nurse direction of LPN and direct patient care to maintain patient medical stability within the Neuro Transitional Center in accordance with Residential Care Facility guidelines. Delegates, directs, oversees and provides developmentally appropriate patient care. Job Description: Essential Functions: Provides nursing care using a Family Centered approach. Assesses individualized patient needs. Communicates patient care needs and priorities to assigned Licensed Practical Nurses (LPNs), Patient Care Assistants (PCAs), Unit Coordinators, and Allied Health personnel. Plans, implements, educates and evaluates care using Evidence-Based Practices.
