Job Summary The RN Case Manager I is a registered nurse responsible for coordinating the care and services for an assigned unit-based patient population. The RN Case Manager I promotes quality of care, supports effective resource utilization, and facilitates the patient's progression of care to proactively plan for the patient's safe and timely discharge to the most appropriate level of care. The RN Case Manager I serves as the key resource for the patient's multi-disciplinary care team and works collaboratively with all members of the patient's health care team and patient representatives to achieve desired clinical, financial and quality outcomes.
