Clinical RN Liaison/GIP Hospice at SRC

Under the direction of the Administrator and Clinical Manager, the Clinical Nurse Liaison RN Hospice represents Yale New Haven Health System (YNHHS)/Yale New Haven Health - Health at Home in system hospitals and the clinical community by initiating and maintaining relationships with referral sources, patients and families and provides patient evaluations and patient care as needed. EXPERIENCE Minimum of four (4) years nursing experience in an acute care setting with at least one (1) year in Hospice required. Minimum of one (1) year Home Care and Hospice GIP experience preferred. LICENSURE Registered Nurse with a valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day.
