FLSA Status Non-ExemptJob Role Summary The Bereavement Coordinator is responsible for coordinating the care for families who have experienced a pregnancy and/or newborn loss. The role includes management of inpatient services offered to the family and outpatient grief support. This position is also responsible for tracking the documentation provided by the OBGYN team of providers regarding pregnancy losses that result from medical terminations, stillbirths, miscarriages and elective pregnancy terminations complications. The Bereavement Coordinator, through collaboration with the OBGYN provider teams ensures that the legal documentation is completed and submitted appropriately and in accordance with the Indiana State Law regarding Termination Pregnancy Reports (TPR’s). Essential Functions and Responsibilities • Ensures accurate and thorough documentation and timely submission of Termination Pregnancy Reports (TPR’s) to Indiana State Department of Health. Vital Signs & Observations: Monitor and record vital signs, daily care tasks, and fluid/food intake; promptly report any physical or behavioral changes to the unit LPN or Charge Nurse. Mobility & Transfer Assistance: Help residents transfer safely, walk, and position comfortably using proper body mechanics and equipment. Companionship & Engagement: Engage residents in meaningful daily activities, conversations, and social routines that promote cognitive stimulation and emotional well-being. Team Guidance: Provide clinical direction, mentorship, and support to Resident Care Assistants (RCAs) / CNAs on the Memory Care floor. Family Communication & Care Planning: Collaborate with interdisciplinary care teams and maintain empathetic, clear communication with families regarding their loved one's care plan and progress. Qualifications Licensure: Active, unencumbered license as a Licensed Practical Nurse (LPN) in the State of Indiana (or compact state equivalent). Certifications: Current CPR / BLS certification required. Experience: Prior LPN experience in Assisted Living, Memory Care, or Long-Term Care is highly preferred. Experience working with Alzheimer’s or dementia care is a huge plus!
