Job Summary Acute Rehab Unit Registered Nurse (Night Shift) provides direct nursing care to patients with temporary, progressive or permanent physical disabilities, trauma and or chronic illness. Create and implement a written plan of care that addresses the specific needs and goals of each individual patient, making referrals to other professional services as required. Modifies the plan of care based on reassessment of the patient including observed patient response to treatment/care and evaluation of patient's progress toward goal completion and delegates tasks to team members, as appropriate within their scope of practice. Observe, document, address, and implement planned interventions/care, communicating the plan to all members of the team through accurate and timely documentation in the electronic medical record and, when needed, through verbal reports. Documents accurate patient information in compliance with policies and regulatory requirements. Provide patient care through the performance of selective procedures that include but are not limited to IV starts and restarts, blood administration and phlebotomy tasks, answering call lights and serving as testing personnel under CLIA1988 regulations as they relate to point of care testing. Administer medications, accurately and timely according to physician orders and organizational policy and procedure. Maintains knowledge of medication actions and side effects, recognizing allergic reactions and documenting patients’ response to drugs. Successfully transition patients, communicating the most current plan of care, through walking rounds and providing accurate, timely report to incoming staff. Advise physicians, other disciplines, family/significant others of changes in patient's condition or abnormal findings. Communicate clearly and concisely. Provide clear, specific educational and discharge information to each patient and/or patient family member. May precept new employees and oversee other members of the care team. Job Responsibilitie s Implement planned interventions/care as directed and supervised by the Registered Nurse and within the scope of the Indiana Nurse Practice Act requirements for LPN's. Administer medications, know their actions, and side effects, recognize allergic reactions and document the patient's response to drugs. Monitor, regulate, change, and discontinue intravenous fluids. Answer patient call lights for any patient and make frequent rounds on all assigned patients to determine condition and needs. Communicate changes in patient condition to the Registered Nurse and other members of the healthcare team. Provide patient education and explain the procedures, medications, and treatment to be administered to gain cooperation and ease apprehension. Actively work to ensure patients’ rights are identified and met for every patient. Collect data as directed by the Registered Nurse assigned to the patient and defined by department policy. Performs registration and scheduling activities per organizational and departmental procedures. Maintains EMR files and performs general front office functions as needed. May perform computerized physician order entry, only if directed by assigned physician. Expected to precept current MA team members or MA Externs as requested by leadership. AHA Basic Life Support for healthcare workers certification required. Other advanced life support certifications may be required per unit/department specialty according to patient care policies. High school diploma or equivalent required. Graduation from a Medical Assistant program preferred. Current certification in medical assisting required from America Association of Medical Assistants (CMA), American Medical Technologists (RMA), National Health Career Association (CCMA), National Center for competency Testing (NCMA), and/or American Medical Certification Association (CMAC). Knowledge of OSHA, CLIA, and ADA required. Knowledge of ICD-10 and CPT coding required. TB validated (to administer or read) or validation obtained within 6 months of hire preferred. Knowledge of Business Unit Quality Programs, Meaningful Use and Standard Work Programs and concepts within 6 months of hire. Knowledge of standard laboratory and medical procedures. Responsibilities: Servant Leadership directs all aspects of decision making Serve as the “Charge Nurse” in the absence of the Executive Director and Director of Health & Wellness Contributes to all aspects of medication management, including but not limited to; orders, administration, storage, documentation and as mandated by the State Regulations and Guidelines as well as Company policy and procedure Assures resident rooms and common areas are always clean and safe Assure that all care partners and medication assistants are familiar with the residents’ individual conditions and are trained to provide the resident services in an effective, professional, and sensitive manner Assist the residents with their self-medication or administration of medications Responsible for limited nursing license daily recommendations and documentation (if applicable) Implements plans of care recommended by the Resident Wellness Director Implements change in condition plans Provides accurate and timely documentation of all care provided to residents Complete resident charting in the electronic health record and assists in completion of assessments as scheduled or with change in condition Completes accident and incident reports as required Maintains positive relationships with residents, family members and responsible parties Be prepared for and manage emergency situations including disasters, fire, and other emergencies Ensures adequate and appropriately trained care partners and medication assistants are available for each shift as scheduled, within company and state guidelines Licensed Vocational Nurse/Licensed Practical Nurse or Registered Nurse license(LVN/LPN preferred) Two (2) years’ experience in a senior living environment preferred Basic First Aid and CPR certification Must successfully complete all company specified training programs Effective communication skills commensurate with professional standards. Ability to communicate effectively with wellness team members, leadership team, residents, family members, vendor partners, referral sources and outside contacts Appearance is neat, clean, and according to dress code.
