Description Northwestern Medicine has an exciting opportunity for either a motivated Physician Assistant or Acute Care Nurse Practitioner to work with our Thoracic Surgery team. This APP will support a hybrid practice model, including surgical first assist responsibilities, and care delivery through the Lung Health Clinic, including review of CT scan results and patient follow up. Schedule: Monday-Friday Essential Functions: Demonstrates clinical expertise and expanded knowledge through patient care activities which may include diagnosis and management of a wide range of health problems. Responsible for health outcome of an assigned patient population. Confers with collaborating physician and other health professionals in area of clinical expertise. Develops appropriate educational systems for patients, families and staff in areas of expertise. Mentors graduate students in aspects of advanced practice and patient care. Responsibilities : Conducts comprehensive appraisals of patients' health status by taking health histories and performing physical examinations in clinical area. Provides direct clinical care to patients. Prescribes/orders medications, therapies; orders/interprets laboratory tests, x-rays and procedures according to clinical privileges. Evaluates patients to determine their need for further health care. Provides comprehensive teaching to patients and families. Documents in the medical record all care and education provided and/or procedures performed for which the Licensed Advanced Practice Provider is responsible. Refers to other disciplines as appropriate. Consultant to other disciplines in areas of expertise. Integrates research based knowledge and clinical expertise in decision making. Exemplifies a professional image in appearance, manner and presentation. Advocate for patients, families, coworkers, and the community. Maintains certifications and continuing education as required for licensure and privileging. Serves as a patient advocate and role model for 'Patients First'. Participates in unit/departmental/divisional or hospital committees/task forces as appropriate. Provides comprehensive teaching strategies for patients and their families. Develops and participates in programs to meet continuing education needs of staff. Serves as a role model and preceptor to students. Quality and Compliance Participate in QAPI activities and performance improvement initiatives. Monitor clinical outcomes, documentation quality, and compliance with agency standards. Support implementation of corrective action plans and quality improvement measures. Assist with survey preparation and regulatory readiness activities. Cross-Functional Support Maintain competency in Clinical Review Manager responsibilities. Assist with review workflows during volume fluctuations, staffing shortages, or operational needs. Collaborate with Clinical Review Managers to improve documentation quality and clinical outcomes. Productivity Expectations Primary responsibility for management of approximately 30-40 clinicians. Clinical teams with fewer than 30 clinicians may require: Additional visit productivity, or Assistance with Clinical Review Manager workflow. Therapy Clinical Field Managers are expected to maintain 24-32 visit points bi-weekly when applicable. Qualifications: Current unrestricted RN, PT, OT, or other applicable professional license. Minimum three (3) years of home health experience preferred. Supervisory or leadership experience preferred. Ability to manage a multidisciplinary team in a fast-paced environment. Proficiency with EMR systems and reporting tools. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. All transfers to Rush University Medical Center or Rush University Medical Group from other Rush entities must have an earned nursing degree at the baccalaureate level or higher prior to the transfer. Chief Nursing Officer must provide approval for any RNs with ADN degree hired after 10/22/2020, based upon unit need. ADN RNs hired after 10/22/2020 must show continued enrolment towards a BSN degree with a signature of contract stipulating a BSN degree must be completed within 3 years of hire, Failure to do so will result in termination. ADN RNS are not eligible for leveling to RN3. Current CPR Certification. Analytical ability. Communication skills in order to lead and teach. Physical Demands Ability to walk throughout the Medical Center and to be standing or walking most of the designated shift. Ability to lifting or carrying objects 35-40lbs. and lifting and positioning patients, some of whom may exceed 300 lbs. Assesses patient safety needs and implements appropriate strategies to minimize risk during clinical visits, telephone encounters and electronic communications. Recognizes changes in patient status and consistently responds effectively to optimize patient outcomes. Recognizes patterns and problems in a specific patient population that require evaluation.
