Overview Under direction, to perform work providing primary care services, assessment, treatment, care planning and medical evaluation, to patients within the scope of their license to practice in the State of Indiana in a physician office practice setting. Responsibilities PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions) Obtains patient consent for care and ensures patient conficentiality, gathers medical history, information on chief complaints, vital signs, and health risk factors and assesses disease conditions. Provides patient care based on practice guidelines, standards of care, physician instruction and federal/state laws and regulations. Executes prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient response. Performs a complete physical exam, including the complete system on patients identifying abnormalities and changes, and documents findings in the patients chart and relays pertinent information to the physician. Provides leadership and direction for the program and office staff, and mentors office staff , providing leadership and direction. Supports the activities of the physicians through consultation and direct patient care activities in physician office settings. Qualifications JOB SPECIFICATIONS(Minimum Requirements) KNOWLEDGE, SKILLS, AND ABILITIES Demonstrated knowledge and skills necessary to provide care appropriate to the adolescent, young adult, middle adult and geriatric patient served within the service line. Demonstrated knowledge of the principles of growth and development over the life span. Demonstrates a high degree of independent action, constantly seeks new information and sources. Professional leadership evident through membership and active involvement in the professional community and organizations. EDUCATION Graduation from a recognized college or university as a registered nurse. Provide IV and IV-site care as permitted by state regulations, licensure, training, and facility policy. Communicate effectively with residents, families, physicians, nursing leadership, and other members of the interdisciplinary team. Participate in shift report and resident rounds and ensure important information is communicated between shifts. Help maintain a warm, calm, respectful, and resident-centered environment. Ensure nursing staff follow established safety procedures and infection-control practices. Protect resident privacy and maintain confidentiality of all resident and health information. Report incidents, concerns, or suspected inappropriate disclosure of resident information in accordance with facility policy. Perform other duties as assigned. What We’re Looking For Current, unencumbered Licensed Practical Nurse (LPN) license in the applicable state. Current CPR certification. Ability to communicate clearly and professionally with residents, families, and team members. Strong clinical judgment and the ability to make appropriate decisions within the LPN scope of practice. Ability to provide leadership and support to nursing assistants throughout the shift. Knowledge of nursing practices, documentation requirements, and regulations applicable to senior living or long-term care. Long-term care, skilled nursing, assisted living, or senior living experience is preferred, but not required. A team-oriented attitude with a genuine commitment to providing excellent resident care. Physical Requirements This position requires the ability to move throughout the community during the workday and perform the physical functions associated with resident care. The employee must be able to safely push, pull, move, and/or lift at least 25 pounds and may be required to assist residents during emergency situations.
