Description The Assistant Nurse Manager of ID (Infectious Disease) Transplant / Ophthalmology / Ambulatory Float Pool is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. Assists in the validation of staff members round hourly on all patients by direct observation and patient feedback. 5. Job Details St. Francis Hospital Critical Care Advanced Practice Provider (APP) is a valuable team member. Primary responsibility is the management of critically ill and complex (multi-organ) patients in an intensive care setting. This setting includes the management of patients in the Cardiothoracic ICU (CTICU), Surgical ICU (SICU), Medical ICU (MICU) and Intermediate Care Unit (IMCU). The APP is under the direct supervision of an in-hospital 24/7 Critical Care board certified Intensivist program. The APP performs physical exams, collects and documents data, conducts approved diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, and provides direct patient care services in a specific ICU. Responsibilities: Provide direct clinical services for critically ill adult patients with multi-system disease /failure, according to established protocols and evidence-based clinical practice standards. Obtain patient health histories and complete physical examinations. Document findings, noting pertinent normal and abnormal findings. Order and interpret appropriate diagnostic and laboratory tests. Explain the necessity, preparation, nature and anticipated effects of procedure(s) to patients, patient's family, staff, and healthcare interdisciplinary team members. Perform approved therapeutic or diagnostic procedures based upon patient's clinical status. Formulate and implement a treatment plan for acute illness management in collaboration with the designated primary supervising physician and/or the intensivist. Utilize prescriptive privileges as allowed by NY licensure. Assess patient for change in status and institute appropriate interventions. Initiate necessary emergency interventions to stabilize patient. Participate in our Clinical Education and Science Simulation center for onboarding and strengthening of both didactic and procedural skill sets.
